DO NOT send your loved one here. Despite the 5-star signs and Facebook photos, Kerens Care Center is a 2-star facility on medicare.gov. We moved our family member out after 2.5 months of constant issues, and the difference at her new facility is night and day. • CARE & NEGLECT: Call lights are routinely shut off and staff leave without assistance but tell the resident they will be right back. On multiple weekends, our relative waited extended amounts of time for restroom assistance, resulting in her soiling herself and being left to sit in it over 40 minutes despite us calling up there to tell them she needed to be changed. • HYGIENE: Showers were completely inconsistent (it took a week to get her first one). Clothes were not changed daily; she was left in the same dirty clothes for days, and once ran out of pants entirely and was left exposed under a blanket. One weekend a staff member plastic bags over her shoes for a shower, leaving them soaked. • STAFFING & TOXICITY: They are severely short-staffed, leading to overworked, rude, and stressed out employees. We witnessed a kitchen staff getting into yelling match with a residents and an aide barking at an elderly woman to "eat your food." One specific aide constantly bullied and gaslit our relative, leaving her with anxiety and scared she hears that persons voice even at the new place. Admins tell you they are not short staffed, but the aides and nurses will tell you the truth. • FACILITY CONDITIONS: The building was crawling with black bugs for over a month; dead bugs littered the halls, and one even fell from the ceiling onto us. Activities are constantly cancelled because directors are pulled to do other chores and the residents suffer and have less socialization due to this. There were no bingo prizes consistently so we purchased and brought prizes often just to see the smiles on their faces. • MANAGEMENT: The Administrator fails to communicate clearly or take accountability. While some positive changes were made in our stay- the laundry staff is so much better now, this facility has a top-down issue. Please tour elsewhere—the extra drive is worth your loved one's safety and dignity. They will tell you one thing and you will observe the opposite. If you choose this facility be sure your loved one has a phone and can communicate to you freely. Be sure you check on your loved one at different times of the day. Watch how your loved ones aide responds to their call light. Be involved and be an advocate for them- you will have to be at this facility. Thank you to the few staff who were kind and worked their tails off, but the overall treatment our loved one received was not quality and common courtesy, human to human was not there. She was treated as an annoyance by many of her aids and her self esteem suffered due to this. She did LOVE the food, but that is simply not enough. Regardless if you are private pay or medicare/medicaid- you can go to a better facility just 20 minutes or so away.

I am deeply disappointed with the level of care and professionalism at this facility. There appears to be a serious lack of accountability and communication from staff and management. Residents are too often left sitting in soiled clothing for extended periods, which is unacceptable and degrading. Personal laundry is frequently lost, and scheduled activities are regularly canceled without explanation, leaving residents neglected and isolated. Families deserve transparency, compassion, and consistent care for their loved ones, and unfortunately this facility has not demonstrated those basic standards. I strongly encourage anyone considering placement here to ask detailed questions and closely monitor the care being provided.
