In my family’s experience Jacksonville Health and Rehab is a horrible place to entrust your loved ones with… It’s a filthy smelly place that only cares about money, not your loved ones well being… They didn’t give our loved one a bath or change their clothes for 4 days and most of his clothes were lost or stolen then they told us to read our contract because they weren’t responsible for lost or stolen items… They probably bilked the insurance company out of $100,000’s of dollars for a 2 week stay and then acted like thugs when asked to replace a $100 worth of clothes!!! They called me on Thanksgiving Day to inform us that our loved one lost an appeal for additional days and if we didn’t come and get them before the next day it would be $450 a day until we did come and get them! It’s not about the $100 worth of stolen clothes… It’s about entrusting your loved ones with an institution that advertise looking after and treating your loved ones with respect, dignity and utmost care but instead, leave them laying there in their filth for four days( probably longer if we hadn’t complained )😡… One of the nurses told us they were shorthanded over the holiday and no one on that side got a bath or change of clothes for those four days😡 Older people are treated horrible and it’s a shame that our government allows places like Jacksonville Health and Rehabilitation to get away with such horrible treatment to the elderly!!!

Ok, this review comes several months late. But my step dad had a heart attack and he was no where ready to come home. His whole personality changed and I didn't know what to expect when the doctor discharged him. It was recommended that he go to rehab because he was a fall risk on top of his mental change. He was supposed to stay 3 weeks but refused and went home after a week. It was nothing against the staff but he wanted to be home. The nurses and staff here were great. They answered all my questions and took great care of my dad. I have no complaints and would recommend them to anyone.

They have the worst staff I've ever seen. My 82-year old mother was overdosed on medication and the staff refused to call for help. My brother called the ambulance himself when he noticed something was wrong and when the staff was alerted they refused to do anything. On another occasion my mom was left in her wheelchair after therapy. When she asked for help getting back in bed the side told her to do it herself. If she was able to do it herself she wouldn't have asked for help. Three people total were in and out of the room within a few minutes and all refused to help her. When she used the call button to ask for a pain pill she was told she used her call light too much and to stop bothering them. No pain pill. This place is the worst!

My mom fell and broke her hip November of 2025. She went to Jacksonville Health and Rehab for 20 days. Mom was well cared for and received excellent therapy to help her gain her strength back. So many of the care givers too time to show her attention. Kim on her first evening made sure she had supper and heated it for her. She helped mom settle in. The facility was clean and no odors were there. The meals were adequate and she requested boiled egg for breakfast and they made sure she received one each day. We were there over thanksgiving and mom was able to leave and join us at home for dinner. He holiday decorations were lovely and there was a very homey atmosphere. Our family would highly recommend their service. Five stars.

My aunt has been staying here temporarily while she recovers. I walked in & immediately noticed it not only looked so clean & pretty it also smelled the same.i have been in several places like it about a half dozen & I worked at a group home this place is cleaner than any other I have visited or worked but most importantly the staff are so kind and on top of treating their patients I witnessed while drinking coffee residents not only know their staff well they like them and act like a family this is definitely THE place I would want any of my loved ones or myself if necessary
