We visited a friend today. The staff was very friendly and helpful. It was nice to see our friend happy and visiting with the other residents. The facility was clean.

I’m so thankful that my husband is receiving the best care, and everyone at Hawthorne is very loving to him. We couldn’t be happier. ❤️❤️❤️ The entire nursing staff are amazing 😘😘😘

I have tried now over an hour and a half and over 30 attempts to reach my father this evening. No one will answer the phone. One would expect compassion, some empathy, consideration and possibility a bit of professionalism or at the very least a tinge of understanding coming from people working in this type of facility, that people need to contact their family members!!! My sister that lives out of the country tried for four days before someone answered the phone. This facility is not one I would recommend and hate that my father is here. It is very unkempt, very basic and beyond outdated. It’s a tragic situation and all the more reason that I need to speak to him and can’t due to subpar employees, that simply don’t care, that should Not work in a facility that requires Care! I don’t know who to contact or where to report this facility to but will do my best to find out and report it. Maybe they’ll revamp, get quality help or be ordered to shut down. This should not be tolerated

The 1st night my Dad was frozen in his room to the point he signed himself out of rehab the next day. The staff seemed like lovely people, but my Father is recovering from a stroke and a seizure. Now I have him at home without rehab. He keeps blaming the hospital for missing the stroke 3 weeks ago. So not this rehabs fault. The staff had some amazing people there. I wish I had the chance to get to know them. He also said he wanted more water. They did have refrigerators in rooms for patients. Again, we did 2 nights 1 day of rehab. They switched his room after the room was too cold. But his mind was made up. They did try to go above and beyond.

Our family had a very concerning experience at this facility. My grandmother was admitted following a hospital discharge after a fall in which she hit her head and injured her shoulder. Despite this, neither issue was evaluated or addressed for more than a week, and only after our family repeatedly raised concerns with staff. She required a two-person assist for toileting, yet staff frequently refused to help her, leaving her in soiled briefs. When assistance was provided, the treatment was often lacking in compassion and professionalism. The facility was fully aware that our family holds power of attorney and health care surrogate authority and that we planned to relocate her closer to home. Despite this, care-plan meetings were held without our involvement. Additionally, attempts were made to pressure her into signing documents and transitioning into their assisted living program, which we found deeply inappropriate. Also, we needed a copy of her ID to take to the post office to close her PO box, and the Social services director Joyce treated us like we were criminals. Finally, while she was placed in a skilled nursing unit and billed as such, the level of skilled care expected did not appear to be consistently provided. Based on our experience, I would strongly encourage families to be vigilant, ask detailed questions, and closely monitor the care of their loved ones if considering this facility, and please find somewhere else to take them.
