About Greenhaven Healthcare Center

Greenhaven Healthcare Center is a skilled nursing facility located in Sacramento, CA. The facility is closely integrated into the community, partnering with local hospitals and community groups to help ensure a streamlined and quality stay for residents. Its smaller size allows staff to provide more individual, person-centered care and attend to the details that matter during a resident's stay.

Personalized plans of care are developed by staff in conjunction with the resident and/or family, and with guidance from the resident's physician, with the goal of helping residents achieve their highest practical level of functioning by combining lifestyle preferences with sound medical treatment.

Clinical services include a team approach to weight management, certified wound care staff who assess and treat wounds, and IV therapy to support recovery. Rehabilitation services include physical, occupational, speech, and IV therapy delivered by a team of qualified rehabilitation professionals, with the goal of helping residents regain health, independence, and quality of life to their highest ability.

Rehabilitation programming covers training in mobility, gait stability, ambulation, posture, and positioning; custom exercise programs to re-educate muscles and improve coordination; specialty programming and technology for balance and fall prevention; joint and soft tissue mobilization; education and training in dressing, bathing, eating, and grooming; activities for memory, orientation, and cognitive integration; adaptive techniques and equipment; and exercises to reduce the effects of arthritis. A therapeutic environment with a full kitchen, bathroom, and bed supports activities of daily living re-training, and speech therapy programs focus on recovering speech, language, and memory skills, strengthening communication, and oral muscle strength for speaking and swallowing.

Recreational and social activities include field trips, cards, arts and crafts, reading, television, hairstyling, bingo, and outdoor walks when weather permits, with residents encouraged to participate in as many or as few activities as they like. Respite care is also offered as a temporary relief option for caregivers and families caring for people with disabilities or other special needs, including chronic or terminal illnesses.

Greenhaven is licensed by the State of California and is an approved Medicare and Medi-Cal provider, with many services covered by various commercial insurance plans; its Business Office is available to assist with questions about insurance coverage, Medicare eligibility, or monthly bills.

I am extremely disappointed with the care my loved one received at this facility. Throughout his stay, it often felt like the nursing staff and CNAs were inattentive, primarily checking on patients only during meals, medication times, or scheduled showers. There was little reassurance that patients were being properly monitored in between, which is deeply concerning for anyone trusting a facility with a vulnerable family member. I was also troubled by how patients were treated. At times, staff appeared rough in their handling, which came across as lacking compassion and patience—qualities that are essential in a care environment. The condition of the facility itself raised additional concerns. It appeared outdated and unclean, contributing to an overall sense of discomfort and a lack of confidence in the quality of care being provided. Communication was another major issue. Attempting to reach a provider was consistently frustrating—calls were often transferred multiple times and, in some cases, disconnected without resolution. It was especially difficult to get in touch with the physician assistant, who seemed to be overseeing much of the care. I am also concerned about what appeared to be an over-reliance on medication rather than attentive, hands-on care. The absence of an on-site physician further raised questions about the level of oversight and medical support available. Prior to arriving at this facility, my family member had been improving. Unfortunately, his condition declined during his stay, which has been incredibly difficult to witness. Overall, this experience has left me upset, frustrated, and deeply concerned about the standard of care provided here. I would strongly urge others to carefully consider their options before placing a loved one in this facility.

Absolutely horrific. The level of incompetence across the board is astounding. The kitchen staff does not know what a diabetic diet is, the admissions staff took 8 hours to admit the patient (it was midnight, I left, maybe it took longer) In the meantime, a diabetic needs their meds. But you don't get them until admissions is complete. The place is dirty. There are no supplies available. The TV is stuck on QVC and the remote is broken so you can't turn it down or turn it off. Apparently only one CNA for the entire wing, so you can call for assistance, but you won't get it. The place was horrific on every level. Don't ever let your loved ones go there. They are not safe. Very disorganized, no one seems to have basic problem solving skills, no one knows how anything is done, its dirty and run down. Avoid at all costs.

Dear Angelina, I am the daughter of your resident, Mr. Fully S. at Greenhaven Healthcare Center in Sacramento, California. I am immensely grateful to you and everyone at Greenhaven who have provided compassionate care to my father ever since he was admitted in December. My father went through his life’s darkest and toughest moments for intensive antibiotics and stage 3 sacral bed sores wound treatment. You and your team have remained a beacon of hope and care for him. I hope my father will continue receiving medical care, physical therapy, and nursing care at Greenhaven. I visit my father every now and then since I am from out of town. It is gratifying to see his health being and wound care has been improved slowly. Your kindness and compassion make Greenhaven a better place for my father. Greenhaven management team and nursing staff are truly the unsung heroes of healthcare and elder care! Graciously yours, Virginia

Because I had Gout and couldn’t stand on my feet and walk very long. I was discharged from Greenhaven Healthcare Center to go to my house where I happen to live alone, I would have had no one to help me get in my wheelchair to maneuver around my house, I also would have had no one to help me get to the toilet , to make food, or to bathe. I was encouraged by the Advice Nurse at Kaiser to go back to the ER at Kaiser and to be readmitted. I believe it was in retaliation for me complaining about the portion size of dinner. Dinner was served at 5 PM and you had nothing else to eat until breakfast at 7 AM. My insurance was paying $625 a day for me to be at this facility. I had to ask for permission in order to Door-Dash something over to the facility to eat for my dinner. It was really a shame that with the amount of money that was being charged to my insurance company for me to be taken care of at Greenhaven Healthcare Center, that I would have to pay for my own dinner not once but twice. Theses are pictures of two of the dinner meals. After speaking to the chef, she said to just ask for snacks when I was hungry again. I Thank her for The offer so that I wouldn’t have to spend more money for dinner.
