For a loved one on a ventilator do not go here. They had no doctor on site. They both were on vacation at the time of transport the whole week we were there. The ventilator kept alarming and they brought out a manual. They then said it was because it was on emergency mode. I looked up code and it was stating a trach leak. Needless to say after 6 days there my dad was rushed to the ER. He had to much co2 build up and had pneumonia. Went to kettering for a week and was already doing better. Then sent him to PAM rehabilitation in Miamisburg, less than 2 week off the ventilator and about to get his tummy peg out. He's doing much better. Kindred sucked too. But with all that said. They did have some really nice nursing staff that did try to help and comfort. And its hot in there and they didn't have enough supplies. Like gloves, sanitizer was not on any wall or room that I seen. For a nursing home you should have wipes, sanitizer in every room along with gloves. Half the staff I seen wasn't even wearing gloves. The place just wasn't sanitary in my opinion.

There is no enough words to express my deepest gratitude to all the members of Gem city rehabilitation/nursing facility. I am quite uncertain about this place at first cuz I have never heard about them before. My greatest thanks goes first to Eric Doan the social worker assign to my mother. Every time I see any problem with the situation, he is always there for us to solve the problems. All the nurses of my mother who is 89 years old are excellent in their profession. Special thanks to Thai(morning shift nurse ) Meranda , CG and another Thai (night shift nurse). I cannot thank you guys enough for taking care of my mother. You guys runs the show over there. Also the CNA that works so hard and very prompt and quick to come in our room every time I get the call light . I will never forget you all most especially Yvette , Jasmine, Brie,Megan, Shantel and Jeremiah. This guys should have a raise for their great job 👏. (Mercedes should know this 😊) And of course thank you to the housekeepers Vi and Bryan for keeping my mom’s room always clean and shiny. This is the place to be for all your loved 🥰 ones. Family has been here for months and the staff have been wonderful. They never complained and never felt like we were a burden when we had to call them almost every half hour to clean our grandma. When the AC went out they were kind enough to give us two fans. House keepers kept the place clean. If you want the floors swept and mopped everyday, you just have to ask and they do it right away. The aids are so fun to talk to. Everyone was kind and supported us through this hard time.

When my mom had a catastrophic seizure that left her gradually declining Gem City Rehab ended up being the best decision I made. It was close to home and when Medicare stopping paying my moms social security was more than enough to over the monthly costs. The staff were excellent and attentive and the facility was kept very clean. The facility is new after being remodeled and the nurse/patient balance felt very well managed and didn't seem like the nurses and doctors were being overwhelmed which allowed for more attention per patient. The social worker Eric Doan was exceptionally kind and helpful. He was always very easy to reach and was able to help facilitate reaching the nurses if there were any communication gaps while they were away from their desk. He also walked me through all of the processes and paperwork needed to make things affordable to help us in the long run.They have a hospice that comes into the facility if that time comes, Sanctuary, and they also did a great job in keeping me updated on my mothers status. They made the nightmare of dealing with my mother's impending death more bearable and gave me the space to grieve and focus on what was important instead of the administrative minutiae. I would recommend them to anyone.

I cannot say enough good things about the care my sister received here. From day one, the Social Worker, Eric, was incredibly helpful, guiding us through every step of the process and making a stressful transition so much easier. He continued throughout my sister's stay to answer questions and support us in every possible way. The nursing staff is top-notch—always attentive, professional, and genuinely caring. Questions and requests were addressed promptly by Ken, Shayla, Dawn and Miranda and their team. A special shout-out to the Physical Therapists, Bob and Melissa, whose patience and expertise helped my sister make amazing progress. I can't believe how fast she has recovered! The facility itself is clean and comfortable, and the PT equipment didn't look over used, which gave our family great peace of mind. We toured several facilities before choosing this one, many of them much closer to home, but this one stood out as the best, and we were not disappointed.
