I just spent 6-7 weeks in the skilled nursing facility at Freedom Plaza Care Center. I had a very positive experience and would recommend this facility if you need care. There were of course outstanding staff members that need to be recognized. The CNA’s that gave exceptional care were: Henri, Lora, Bernard, Heather and George. They were compassionate, and caring and did a good job taking care of me. Then there were the nursing staff that were exceptional and need to be recognized: Debra, Anastasia, Takema, Janelle, Kiana, and Isabel. These ladies always cared about me and my needs. They too were compassionate and respectful and very caring. The resident doctor who I only know as Willie was amazing. He was always very positive and encouraging for patients. I had a wonderful experience and I hope that the above staff members will know how much I appreciated everything they did for me. Thank you. Julie

My mom stayed at Freedom Plaza back in 2024, and from the very beginning, we knew we had made the right choice. Even before her admission, Jeffrey visited her in the hospital and helped ease many of our concerns. The admission coordinators were efficient and made the transition seamless. Once mom arrived, the level of care exceeded our expectations. The nurses, CNAs, and doctors were not only knowledgeable but also compassionate. They always took the time to help my mom understand her condition. The therapists pushed her in the best way possible by encouraging her progress while still being supportive. The social worker provided valuable information about programs that could assist her after her stay, and the case managers made sure she was well-prepared for discharge. Throughout her stay, the staff treated my mom with genuine kindness and respect. Communication with our family was consistent and clear. The facility itself was always clean, and my mom even commented on how much she enjoyed the food. My dad visited every single day, and he always felt welcomed no matter how often he came. What stood out most was that the compassion didn’t stop with my mom. It extended to our entire family, and that meant a lot to us. You can tell this facility has an administrator who takes pride in what they do because the staff reflects strong work ethic and leadership. To this day, my mom still talks about the wonderful care she received which is why I felt inclined to leave a review years later. If you have a loved one in need of rehabilitation, I highly recommend Freedom Plaza Care Center.

Our family would like to thank the staff at Freedom Plaza for taking such great care of our mother while she was in the facility's skilled nursing unit for three weeks. We were all impressed with the level of care, the cleanliness, the quality and variety of food in the dining room, and the willingness of the staff to contact us when questions or issues arose. Special shout out to CNA's Bernard and George, and physical therapist Lance, who were particular favorites for our mom. We also greatly appreciate the guidance we were provided by case manager, Lyrrae, who was instrumental in making Mom's subsequent transfer to an assisted living facility. Great job all around. We would highly recommend this facility!

From the moment of contact, I give this facility a 100 % A+ in all areas. The staff to a person, everyone who entered my room and provided care, was couteous, professional, knowledgable, kind, and attentive. As a retired RN I know all too well the demands on time and having 5 people want somthing "right now". The food was really excellent, the dietician met with me to determine my nitritional needs post ankle surgery. The housekeeping staff was courteous and kept the facility very clean. The physical therapy staff was encouraging and professional, helping me understand my situation and what was needed to gain strength and endurance to heal. The administrators who came to my room to make sure I was being cared for, the doctor / PA, lab techs, activity staff, all over the top with their concern and making sure I had knowledge and access to services. I fear I am leaving someone out, but except for the reason I was there, I was almost glad to be there (smile) and experience the healing environment. My highest appreciation for your care center. Please keep on doing what you're doing -- I will heartily recommend you to everyone I meet. Thank you so very much!!!

It's been a little over 3 weeks now since my two week stay at Freedom Plaza. I was there recovering from a fall leaving me with a broken wrist and a fractured femur. I cannot say enough good things about the entire staff of the facility. Although there may be certain members of the staff that stand out in my memory, there is little doubt that every single one of them is dedicated to the welfare of their patients. From my perspective, the facility ran with a well disciplined order. Freedom Plaza advertises Chef prepared menus. Make no mistake, I experienced some of the finest meals during those two weeks, in or out of a facility. MY SINCERE GRATITUDE TO THE ENTIRE STAFF OF FREEDOM PLAZA ! Jon Puleo Glendale, AZ
