On my visits to my cousin, I have always found everything acceptable. Her caregivers are friendly. Her room is clean. She is clean, and happy.

I would not recommend Forrest Park Nursing & Rehabilitation to anyone considering care for a loved one. The facility appears to have constant staff turnover, which has led to serious communication and accountability issues. During a recent visit, I arrived to see a loved one and was never informed that she had been moved to a different room. I initially walked into the wrong room and had to ask multiple staff members where she had been relocated. No one seemed to know. After finally being given a room number, I went there only to discover she wasn’t in that room either. When I asked staff where she was, no one could tell me. I had driven over 1 hour and 20 minutes to visit her. After calling my loved one directly, she informed me that she had already told several staff members where she would be, but apparently no one paid attention. In fact, it was other residents who ultimately helped me find the correct room. That alone speaks volumes about the lack of communication within this facility. I also attempted to speak with a case worker regarding concerns about her care. I called approximately 13 times and left multiple messages, yet no one ever returned my calls. I also left messages for the Director of Nursing and never received a response. The one bright spot in my experience was Heidi, who genuinely tried to help and was professional and compassionate. She deserves recognition for her efforts. Another serious concern is that staff freely provided the facility’s door code to me and others. This raises significant safety and security concerns for residents and should never happen. Based on my experience, I have serious concerns about the management, communication, and overall operation of this facility. I will be contacting the Department of Health and other appropriate agencies to report these issues. Families deserve better when entrusting the care of their loved ones to a nursing and rehabilitation center.

On Our first visit the room was dirty, pills on the floor, exam gloves on the floor, dust and dirt around the bed. The Roommate’s television was on high volume. We were able to have our father moved to another room. It was quiet, cleaner, and seemed a bit larger. The staff are all pleasant, helpful, and attentive.

The staff was very friendly, starting with the front desk. The person I visited seemed well taken care of. I interacted with two staff members while visiting, and they were both kind and professional. I left very glad that my friend was in a good environment.
