Elevare Care Center

Elevare Care Center is a memory care community in Melbourne, Florida, located on Viera's Space Coast. The community was recently acquired and reimagined from its previous iteration as Market Street Memory Care, rebuilt with a new team and elevated program focused on physician-led care and hospitality.

The community offers specialized memory care with a primary care physician embedded on campus rather than visiting monthly, ensuring residents have a physician they see regularly and acute issues are assessed within hours. Caregivers are trained in validation therapy and nonpharmacologic redirection, meeting residents within their reality rather than arguing with it. Each resident arrives with a Life Story file covering work, family, faith, music, and memories, and care plans are built from this foundation before day one.

Dining is chef-led with seasonal, restaurant-style menus built from fresh Florida produce, with each dish plated beautifully and adapted for every stage of memory. The residences feature light-filled private and companion suites in a secured community with wander-safe design that feels invisible to residents. Grounds include landscaped areas with shaded, walkable paths and sensory gardens.

Daily life enrichment is structured with intention, including morning movement, music, reminiscence, art, and pet therapy scheduled by therapeutic intent, along with evening activities like karaoke. Every waking hour has purposeful structure rather than activities simply filling a calendar. A nurse is on site every shift, including weekends and holidays.

The community is owned and operated by Elevare Capital Family Holdings, a Florida-based healthcare platform, with clinical leadership including a licensed occupational therapist and practicing primary care physician overseeing the onsite medical model.
