Eastgate Manor Nursing & Rehabilitation is a nursing and rehabilitation community located in Washington, Indiana, offering short-term rehabilitation, long-term care, respite care, hospice care, and outpatient therapy services.

Short-term rehabilitation is designed for individuals recovering from stroke, cardiovascular difficulties, orthopedic surgery, and other debilitating conditions. The program includes physical, occupational, and speech/language therapies along with interdisciplinary support from doctors, nurses, therapists, and social workers. The community offers a Road to Recovery program where the interdisciplinary team, patient, and family meet within 72 hours of admission to establish discharge goals. Services encompass wound care, rehabilitation screenings, fall prevention, mobility training, and strength-building wellness and aerobic conditioning programs.

Long-term care is provided for residents who need assistance with daily tasks and can no longer be cared for at home. Licensed nurses and specially-trained health care aides deliver care under physician guidance, with services including social services, activities, physical therapy, occupational therapy, and speech therapy designed to maintain residents' independence. The community accepts private pay and Medicaid for qualified applicants.

Respite care provides short-term stays for elderly family members, allowing home caregivers time to travel, work, recover from illness, or handle other responsibilities. The community assesses the needs of respite guests and can accommodate extended care if necessary.

Hospice and palliative care are offered in partnership with local hospice companies to provide comfort, pain relief, and support for those facing terminal illness.
