My family member stayed at Deerfield Beach Rehabilitation, and we were very pleased with the care they received. The staff was kind, attentive, and professional throughout their stay. The facility was clean, and the rehabilitation team provided excellent support. Thank you to everyone for helping make their recovery a positive experience.

(3rd review) DEATH TRAP……DO NOT SEND YOUR FAMILY HERE. THE STAFF DOESN’T CARE NOR DOES ACHA. I’ve filed 2 complaints with ACHA and they have seemed to never find anything wrong with that filthy, odorous facility. It’s their money machine of course they won’t shut them down. They need to clean up house and start fresh. Fire everyone starting from the top all the way to the bottom of that dirty barrel. I’ve seen DOGS get better care than the human beings in this facility. Disgusting facility disgusting employees! That place needs to be under the ground. (2nd review) I am writing to formally report what was, without question, the most disturbing, negligent, and dangerous healthcare experience my family has ever encountered. The care—or lack thereof—that my mother received at your facility directly resulted in a life-threatening medical emergency and could have cost her life. My mother was left with an indwelling catheter that appeared not to have been properly flushed, cleaned, or maintained for an extended period of time. The condition of this catheter was visibly unsanitary and showed clear signs of neglect. Within two days, my mother became septic. When I raised immediate concerns to the night nurse, Marcia, and explicitly stated that the catheter appeared infected and required urgent attention, my concerns were completely dismissed. I was told that my mother “just needs to drink more water.” No assessment, no escalation, no intervention—just dismissal. At that point, my mother already had a UTI, which subsequently triggered a recurrence of MRSA that entered her bloodstream. As her condition rapidly deteriorated, I was forced to raise my voice and demand that an ambulance be called. Had I not advocated aggressively, I am convinced my mother would not be alive today. Once in the Emergency Room, the hospitalist physician stated plainly that bringing her in saved her life and that this situation “could have been deadly.” The physician further advised me not to allow my mother to return to your facility if staff are unable to recognize such obvious and dangerous changes in a patient’s condition, calling it a major red flag. My mother was admitted and remains on intensive antibiotic treatment at a different facility. She is now recovering from an infection that was entirely preventable. This is my second formal complaint regarding this facility. Additionally, after informing others that my mother was placed in your care, multiple individuals independently warned me not to allow her to stay there, citing repeated patterns of neglect and lack of concern for patient safety. That alone speaks volumes. This facility is not just negligent—it is dangerous. Based on my experience, it is a death trap for vulnerable patients. I have photographic evidence documenting my mother’s infection and the condition of her catheter. While this platform does not allow uploads, I am prepared to provide this evidence immediately upon request. I expect this complaint to be taken seriously and investigated thoroughly. What happened to my mother is unacceptable, unethical, and a gross violation of basic standards of care. No family should ever have to fight this hard to keep their loved one alive. (1st review) I am writing to formally report the extremely unsafe and unsanitary conditions my mother was subjected to upon her admission to your facility yesterday, following her transfer from Broward Health Imperial Point. Upon arrival, my mother was placed in a room that was visibly contaminated with feces. Fecal matter was present on the floor, toilet seat, sink, guard rail, toilet paper, and door handle. These conditions posed an immediate health risk to my mother. I promptly informed the nurse on duty, who personally cleaned the room herself. When I asked why environmental services had not addressed this, I was informed that the cleaning crew leaves at 3:00 p.m. It is deeply concerning that in a healthcare facility—already understaffed—nurses, PCAs, and LPNs are expected to clean biohazardous waste while also caring for over 40 patients.

I feel compelled to share my experience with Deerfield Beach Health and Rehab because the conditions there are extremely concerning. The facility appears severely neglected and rundown. There are visible signs of mold, constant water leaks, and broken air-conditioning units that look like they’re literally coming off the walls. Water is frequently left sitting on the floors, creating clear safety hazards for both residents and staff. For a facility responsible for caring for vulnerable people, the physical condition of the building is alarming. The atmosphere inside is just as troubling. The environment feels depressing and chaotic. Residents can often be heard crying out that they want to go home, and I have even heard someone yelling for the police. It is heartbreaking and deeply unsettling to witness. My godfather currently resides here and frequently shares how poorly some residents are treated. He has described certain nurses as rude, dismissive, and inattentive. Hearing how residents are spoken to and treated only adds to the concern about the overall level of care being provided. Because of the seriousness of what I have personally observed, I have reported these conditions to the state. Families trust facilities like this to provide safety, dignity, and compassionate care for their loved ones, and what I have seen raises serious questions about whether that standard is being met. Additionally, I’ve noticed that a significant number of staff members have recently been let go, which raises even more concerns about the stability of the facility and the quality of care residents will receive going forward. Given the conditions and the turnover, I honestly would not be surprised if the facility ends up being forced to shut down. No one’s loved one should have to live in an environment that feels unsafe, neglected, or disrespectful. Based on what I’ve seen and heard, I would never recommend this facility to anyone looking for care for their family member.

This place has been terrible with customer service. My dad was sent there for rehab after his knee surgery. While he was there, trying to get a hold of his nurse or whoever was caring for him was impossible. Everytime I call I could not get in touch with anyone. My dad went 10 days without a bowel movement, even after he mentioned that to them, and when I finally got a hold of someone they suggested milk of magnesium... Anyone in the medical field would know at this point the milk of magnesium will not do anything. I had to suggest an enema or glycerin suppository.. After they gave it to him no one followed up on him. Since he was on a machine for his leg, he couldn't move, even after he pulled the call light and screaming for help for nearly two hours no one came to check on him to the point he soilded himself. When tried to find out what happened all we got is we were busy. Also, he had a follow up appointment with his orthopedist, which I was reassured the would take him to prior to him going to this rehab place, but no one ever did and it send no one knew anything about that and he missed his doctor's appointment. This place promised a lot but don't deliver. I feel like this is Medicare fraud of they're telling people beforehand of what they're going to do but don't do them. Don't send your love ones there .
