Forest Hill Healthcare & Rehabilitation is a skilled nursing and long-term care facility in St Clairsville, Ohio, licensed for 88 residents.
Family experiences at Forest Hill are notably divided. Several families praised individual staff members for genuine care and attentiveness, one person highlighted how a night aide "went out of her way" during a difficult moment, while another credited a nurse named Jordan for taking time to explain care with evident compassion. The physical therapy department and overall cleanliness drew positive mention. However, multiple families reported serious concerns: delayed response to medical needs including untreated urinary tract infections, residents left unattended for extended periods, and difficulty reaching nursing staff and leadership by phone. One family described a patient eating breakfast without utensils after being unable to reach a call button, and another reported a hospitalization after a 13-hour stay.
The facility holds a 3.4 out of 5 Google rating based on 21 reviews. Prospective families should weigh the positive accounts of individual staff dedication against the documented gaps in responsiveness and supervision that appear across multiple independent accounts.

The nursing staff took very good care of me during my stay. I had one very bad night and the night aide Destiny went out of her way to take care of me and not make me feel bad about anything. I would highly recommend Forest Hill.

If leaving a patient on a bedpan for 3 hours is okay with you this is the place for you. If serving food with no utensils and the patient not being able to reach the call button which then had to eat breakfast with hands is okay with you this is the place for you. These will be reported to the state. Along with a few other things I will be addressing!!

I’ve never been in one of these places. My first night I had a melt down. Jordan was my nurse. She took time to explain what was happening and really cared. I’ll never forget her passion.
