We were initially nervous about Constellation based on the reviews, but our experience was entirely positive. Julie, our hospice nurse, was kind, caring, and extremely knowledgeable, and Shamoun, a nurse's aide who helped with personal hygiene was equally wonderful. We had less contact with some of their other folks, but they were OK as well.

Gladys is an Excellent nurse. She was very informative and answered all of my questions.

Regrettably, I failed to conduct thorough research and read reviews prior to enrolling my father in this institution. The level of neglect experienced here is deeply concerning. For the well-being of your loved ones, I strongly advise against considering this company. Legal action is being considered.

If there were zero stars I would use it. Don't use this home hospice care unless you hate your mother and want the last days her life to be an absolute nightmare! That's what happened with mine. Hospice workers didn't come when they were supposed to....were rude and had attitude. When they were supposed to be comforting at the worst time in my life....only caused more stress & anxiety.

Hired for home care for my mom after her surgery. Staff sent to the home was very poor quality. Sat around, did nothing for my mom. Aid worker would put TV on in Spanish, which my mom does not speak, fell asleep on the couch during her shift. Would not recommend service, sent email to placement director, never got a reply. Don’t trust this service with your loved ones.

Does Medicare cover care from Constellation Health Services?

Yes. Constellation Health Services is a Medicare-certified home health agency, so Medicare Part A or Part B pays in full for skilled nursing, therapy and home health aide visits when a doctor orders them and the patient is homebound. There is no copay for covered home health visits. Medicare rates the agency's quality of patient care 4.0 out of 5 stars.

Do I need a referral to use Constellation Health Services?

For Medicare to pay, yes: a doctor, nurse practitioner or physician assistant must certify that care is needed and sign a plan of care. Families can call the agency first, and the agency will usually contact the doctor to arrange the order. Ongoing help with bathing, meals and companionship is a different service, non-medical home care, which needs no referral.
