If I could give this place a 0 I would. Do not put your loved one here. My grandmother was here less than a week LESS THEN A WEEK and was not cared for. They hardly came to check in on her, and the reason why they didn’t was because my great aunt was there visiting. Quoted from a very nice cna. REALLY though!! That’s why we are paying you all so we can continue memories and enjoy life with her while we still have her. You take care of her, clean her, feed her, whatever is needed while we sit, visit, watch tv, play games, whatever. You still come in and do your job. I’m not going to do it for you! The first night she was there the cnas didn’t even know she was even in her room. I know this because my great aunt was there waiting for someone to come in the room finally my grandma got tired and fell asleep and my great aunt got up to leave. My grandma couldn’t use the call button and couldn’t speak so how was she suppose to get help when needed if no one was checking on her? The cnas were watching tv and the RN was no where to be found for 2 hours!!! she was in another residence room. What takes 2 hours!!! A NAP!? Plus her son 8 yrs old was at work with her! Playing in a wheelchair. Seriously! So unprofessional!! I get it I have kids too if you don’t have anyone to watch them hire someone!!! Or here’s one for you CALL IN!! Take care of your kid. You are taking care of senior citizens when you are at work. That kid can leave something in the hall and a resident could fall how could you explain that? There are medical supplies, medicine, all kinds of things your kid can get into and she’s not even watching him, no one was. He came in and had a conversation with my grandma and great aunt in the wheelchair and then started dancing. Get a baby sitter. Don’t bring your kid to work. So unprofessional !!! Cnas and the nurses on duty are supposed to be on their feet and doing their rounds making sure our loved ones are safe, and if they need help with anything. My grandmothers feeding tube got clogged. Hummm I wonder why… well I’ll tell you. We had to tell the nurses SEVERAL times the water bag was out of water. CONTINUOUSLY!! Do your flipping job people. Gail a so called RN would say I have other resident I have to take care of right now that can wait. Really… This is important. It can get clogged within minutes lady get off your butt it doesn’t take more then a few minutes to do this, this is going to save someone from having them to go into surgery. Well that water bag issue happened several times. Then we get a call one morning that the feeding tube was clogged she has to go into the hospital. We get to the hospital. Her feeding tube wasn’t even in her! Someone pulled it out! I don’t know if they pulled it out to clean it (wouldn’t put it passed them) or what exactly happened. Do you all know how to flush the feeding tube? Do you know it needs to be flushed every single time of use? Do you know the water bag has to be ran with the food at all times to dilute it? Do you know she has to be sitting up at a 30’ angle or higher at all times when the feeding tube is running or she can aspirate? I will pray for the people that work here because god is the one with all the power. Please please please rethink about putting your loved one here!! We regret putting ours here she will nor any of our other love ones will ever go here again! You all burnt our recommendation.

The new corporate female, Jodi, the old administrator at shadyrest home in Pryor, OK. She shouldn't be in the corporate position she has no clue of policy or labor board policy and procedures. Or the nursing board of Oklahoma, treating publicly, I'm blessed I have no one of my family in this facility.

I've been here 7 months and I'm very satisfied with how I'm being treated. My only complaint is the food. Lunches are great. I started buying my own cereal from Walmart because I could never get the kitchen help to fix me what I ordered. They got stuck on one thing and that's whst I got every day. I'm tired of chicken noodle soup for dinner too. Other than that, I'm happy.

I had a recent after care stay here and for it to be a "nursing home" care facility. It was a pleasure to have been taken care of by the most curtious, friendly, and nice's staff I have ever met. Tended to all of my request in a swift professional manner. Even though the building is well dated, the housekeeping staff did an excellent job in keeping the facility clean. The food service staff served the best home fixed meals. I hope I don't need to have after care in a skilled nursing facility again, but if I do. I hope they have a room available for me. Thank you to the entire staff of colonial terrace care facility. Tedd..

My Mom stayed there briefly. They allowed Avon reps to come in & take orders from my Mother (who had Dementia , she didn't know what she was doing). I walked in & the Avon lady talked my Mother into ordering $390.00 worth of products she didn't want. Taking advantage of elderly people is disturbing and disgusting.
