About CenterWell Home Health

CenterWell Home Health in Orange Park, Florida provides home health and hospice services. The agency serves patients across physical therapy, occupational therapy, wound care, and nursing support delivered in the home setting.

Families consistently praise individual clinicians for their skill and compassion. Patients highlight physical therapists and nurses by name, crediting them with professional care, attentiveness to detail, and genuine patience. One family wrote of a nurse who "showed genuine compassion and patience" when others had not succeeded. Families also note that staff members became trusted figures in their care journey, checking for subtle changes in condition and advocating when patients faced health crises.

Scheduling reliability emerges as a significant concern across multiple reviews. Several families report inconsistent appointment times, last-minute changes, no-shows without notice, and difficulty establishing a predictable weekly routine, issues that persist even after initial orders for supplies and visits. One family noted being "left in the dark" about which nurse would arrive and when, adding stress during an already vulnerable time.

CenterWell Home Health holds a 4.7 out of 5 rating across 119 Google reviews, reflecting strong satisfaction with clinical quality tempered by operational friction.

Toni Parker LPN is absolutely a bad ass Nurse, very knowledgeable has great attention to detail, I would recommended her to anyone. She knew how to pay attention to my concerns and helped me walk through becoming calm before thinking the worst.
