Our family is deeply grateful to the entire team at Spring Canyon Post Acute for the exceptional care my mom received after her femur surgery. Her recovery has been long and difficult, but thanks to your dedication, she is walking again and has finally returned home. Thank you to Naomi and Rolly for managing her transitions between the hospital and the nursing home with such care and professionalism. Thank you to Gloria and Grace from Social Services for guiding us through the discharge process and helping us understand every step. We are also incredibly thankful for Sisi (nurse), Yuxi (nurse), and all the nurses, as well as Rhea (CNA) and all the CNAs whose patience, kindness, and steady support made such a meaningful difference. Your gentle care and encouragement brought my mom comfort during some of her hardest moments, and we are deeply appreciative of each one of you. A special thank‑you to Kim and the PT team, and to Joanne and the OT team. Your expertise, compassion, and positivity helped my mom regain her strength and confidence. Canyon made appointment with a close by suture remove clinic 2 weeks ago for my mom, I was told about this appointment by the clinic(they need me sign paperwork) the night before. Appointment is 8:45am, Canyon said will send my mom to the clinic at 8:15am. I arrived at clinic at 8:20am to sign, but my mom still not there. At 8:40am the clinic called Canyon warning there will be only 10 min late window, if patient not show up at 8:55am, they will cancel and reschedule. So I rushed to Caynon at 8:45 and walk my mom to the clinic in wheelchair, when we back to clinic, it was 8:54 already, barely within the late window. The Clinic told me we are not alone, this rehab always late/reschedule appointments with them with different excuses, recommend me to talk to their director. When I back to Canyon, the guest reception said " you are too rush, the transportation shows up right after you leave", it is true or not, I don't know, but when I go to pickup my mom in person, it is already past the appointment time, if this is called "rush", I don't know what is their definition of " non-rush, normal speed". This actually not the first time to my mom (we only been here 2 weeks now), we had another doctor appointment last Friday, I told them last Monday, and they confirmed transportation arranged, but on Friday morning, they called me saying the transportation guy called sick and need we reschedule appointment. That is a cancer treatment appointment we dont't want to cancel, so they helped us arranged yellow cab to send my mom with we pay for the fee, which is fine, I understood that and appreciated they helped us with yellow cab arrangement. But today if that clinic is not within walking distance and I didn't rush to Canyon to send my mom in person, I am pretty sure we will miss the appointment.

My family and I are grateful to the team and staff at Canyon Springs for their tremendous service and care of my father, which consequently was a great support for our family. My father was hospitalized in June for fractured rib injury from car accident and once again in August for debilitating pain from uncontrolled diabetes. Both hospitalizations required my father to be transferred to skilled nursing inpatient care. We chose Canyon Springs because I received a call from Rolly Hernon when SNF requests were submitted by the hospital case manager. Since I did not know Canyon Springs or the other SNFs, it was very helpful that Rolly reached out directly to me. When my dad was unexpectedly hospitalized again in August and needed to be transferred to skilled nursing care, we already knew to request Canyon Springs. Rolly and the team - Noemie, Grace, Jeng, Choi - at Canyon Springs were always prompt to address any questions or concerns we had about ensuring my dad’s comfort and care. For the 3-week duration of my dad’s second stay, I was in frequent contact with Jeng to coordinate his doctor & specialists visits and monitor his progress. It was reassuring to have Jeng's tremendous support ensure my dad's needs were addressed and progress on track. It was also on daily check-ins during these 3 weeks that my family saw firsthand the hard work and dedication of the nurses, therapists, and all staff at Canyon Springs. On behalf of my dad and our family: Thank you to everyone at Canyon Springs for your service providing health care and care giving! Your care and dedication make all the difference and means a lot to us!
