About Brookside Healthcare Center

Who We Are

Founder and Chairman of the Board Stephen L. Rosedale established the CommuniCare Family of Companies in 1984. As a young man, Steve worked in various healthcare environments and developed a passion for caring. He continued to study the art and science of aging at Brandeis University, where he sparked a close relationship with Professor Morrie Schwartz (of the famous book, Tuesdays with Morrie).

A concept that struck a chord in his heart and has since remained vibrant is the concept of therapeutic milieu. This is the impact of a holistic environment, comprised of people, clinical programs, and physical environments that create a healing environment where a person can thrive. Since then, Steve has been on a mission to build caring communities that cater to the unspoken needs of our guests and residents.

A family-owned company, we have grown to become one of the nation’s largest providers of post-acute care, which includes skilled nursing rehabilitation centers, long-term care centers, assisted living communities, independent rehabilitation centers and long-term acute care hospitals (LTACH).

Each one of our employees primary importance stems from our philosophy: Serving with Pride. We recognize that we are called to reach out with our hearts and touch the lives of others. This recognition instills a great sense of purpose in the work we do each day. As a result, we take immense pride in our opportunity to deliver the highest quality of care. We remind ourselves daily that we have to serve with pride and this energizes us in our mission to, to give the gift of life to each and every guest and resident of the CommuniCare Family of Companies.

Assisted Living Communities at CommuniCare For older adults, aging is a time of decreased functioning and increased dependence. But as with all individuals, their needs are unique. For some, a skilled nursing center would provide more healthcare than necessary. On the other hand, living on one’s own may be too difficult. CommuniCare can help. Our assisted living communities provide a combination of housing, personal care services, nursing and healthcare, all in a supportive living community that promotes maximum independence, privacy and choice for residents who prefer not to live alone, but who don't require 24-hour care. Our assisted living and semi assisted living environment are designed to be welcoming and home-like, while supporting the physical, emotional and intellectual needs of our residents. Choices begin with assisted living accommodations designed for those who prefer privacy, as well as those who don't care to live alone. When available, we have conveniently located our assisted living communities on the same campus as our nursing centers, enabling residents to easily access physical therapy programs, as well as occupational and speech therapy, if it needs to arise. As requirements change and long-term, assisted senior care concerns need to be addressed, our center represents a natural transition within a familiar environment.

I stayed in this nursing home for several years after my stroke. I got along with all of the staff and all my needs were addressed and met with compassion and care. Rikki in admissions is a rock star and always went above and beyond the call of duty.

Brookside has been a great partner to work with our Hospice Team, The floor staff are always helpful and friendly, the house keepers and custodians are fantastic and do a great job keeping the facility clean and neat. The DON Andi does not hesitate to put a hand out to help and goes above and beyond with customer service, her phone is always on. The Admin Stacy is wonderful and responsive. We recently received a call from a family member who lost a loved one there, she reported that Dignity Care Hospice Team was phenomenal and she couldn't have gone through losing a loved one without the team, she also recognized DON Andi at Brookside as well who took the time and looked up everything she wanted to know on the computer amd was so patient. Dignity Care and Brookside make a Great Team!

A long over-due big shout out to the administration, nursing staff and all the wonderful aids. My husband, a Vietnam Veteran, was admitted mid October 2025. From a smiling face who welcomes me each day at sign-in to constant conversations with the head of nursing and to the good folks on the nursing/aides team, my husband has received excellent professional care. It is a testimony to Brookside’s leadership team that has created such a positive environment. It shows each day through the good folks who care for residents with such grace and devotion to them all. It is very reassuring to know that when I cannot be here, there’s an entire team of professionals who always have my husband’s comfort and well- being at the heart of their efforts. I will never be able to thank Brookside enough for all that they are doing to help our family navigate our very difficult journey.

Never. Ever. Ever. leave your loved one here. Ours was there only for a respite stay while we went out of state to check on other family. We arrived to pick him up to find out that no one knew his name (he has dementia and aphasia and can't tell you). He was being called "Carl" and other names. The name tag on the room entrance said a different name entirely. His dentures had ten days worth of dried glue in them, had never been cleaned, and didn't even fit in his mouth correctly. Staff made visible gestures and audible sounds of disgust when touching denture supplies, claiming "eww, no one even has those here". Dad was filthy and all of his bathing supplies were unopened, as were his denture cleaning supplies. Dad smelled so badly of fecal matter and body odor that when we got him home we had to shower him to clean what appeared to be at least a week of old fecal matter from his person. When getting him ready to shower, we noticed the electronic ankle bracelet (tracking device) was still on his ankle. An hour and a half to get him processed to go home and they left the monitor on him. We had to cut it off. His brand new electric razor, in its case with the accessories is also missing, saying they never saw it. Never ever again.

If you have an organization that is looking to do volunteer work the fourth floor of this facility could really use some cheering up. The staff are friendly but completely overwhelmed. Brookside has taken on way too many patients and don't have near enough staff to provide adequate care. The building itself smells terrible and is extremely dirty. Luckily my dad was only here a few weeks. I feel terribly for the patients who live there long term.
