Jaylyne is an outstanding social worker who truly cares about her patients. She is responsive, compassionate, and always goes the extra mile to make sure everything is coordinated smoothly. Her communication is excellent, and she is dedicated to making sure patients receive the support they need without unnecessary delays. I am utterly appalled by the level of negligence and unprofessionalism displayed at this facility. From the moment one enters the building, the atmosphere is profoundly unpleasant. The receptionist is dismissive, which sets a deeply troubling tone for visitors and residents alike. Sadly, the social services director’s demeanor was remarkably abrasive and indifferent. It is deeply disheartening to witness residents left sitting in hallways appearing forlorn and neglected. The environment lacks the dignity and attentiveness that vulnerable individuals deserve. During my visit, I overheard CNAs on the 3–11 shift engaging in excessively loud and inappropriate conversations filled with profanity, which I found entirely unacceptable in a healthcare setting. What disturbed me most was the condition of my loved one. He was completely soaked and appeared as though he had not been properly changed or attended to in an extended period of time. It was absolutely heartbreaking to witness such apparent neglect. Families place immense trust in facilities such as this to provide humane and attentive care, and based on my experience, that trust has been profoundly violated. I will say, however, that the ADON was exceptionally helpful and professional throughout my experience, and Mary from therapy was absolutely wonderful, along with Alisha. Additionally, the social services assistant demonstrated far more compassion, diligence, and competence than the director herself. That young lady went above and beyond in her efforts to provide assistance. Those individuals treated residents and their families with the compassion, dignity, and respect that should be standard in any healthcare environment. Unfortunately, despite the efforts of a few compassionate staff members, I would not recommend this establishment to anyone seeking attentive and professional care for their loved ones.

I wish I could give negative stars. My mother in law has been in this hell of a place for a year and a half. It's been the worst experience. They took her off all psych medicines even though she has multiple mental illness diagnoses, so now all she does is scream and cry almost 24/7. My husband has had the hardest time getting a hold of anyone, and the turnover is so crazy, you wind up never speaking to the same person for long. Every social worker has been completely inept. We tried to get her moved, but they kept dropping the ball on their end, so there she remains. It's honestly infuriating, and it makes us feel so helpless. Now they're claiming the POA that has been on file for a year and a half and has been accepted by Mayo, Baptist, Memorial, and Kindred hospital is not valid and are refusing to speak to my husband (her only son and next of kin) on any medical related items. Also, the website to pay has been done for like 10 months at this point. My husband has to either attempt to call someone (what a joke) or mail a check each month. I would never ever ever recommend this place to anyone unless you hate your loved one.

If you have any other options, use it. My father was in this nursing home for 11 months. There a lot of nice hard working people there, but not all of them. Rooms don’t get cleaned up on regular basis, nurses have way too many patients to do more than just give medication, food is very nutritional poor. Company that own it trying to spend as little as possible, keeping ceilings leaks, rooms in terrible condition, salary minimal. My father was ignored, pain not addressed, bed not changed.
