Zero stars. My grandmother became very ill from this place. They moved her from the 2nd floor to the top floor because she went to the hospital and someone needed the room without consulting family first. She then became ill and passed. It smelled bad on the top floor. We complained to selin the supervisor and another but they did not help. They lost some of her clothes when moving rooms. A overnight staff from another place removed the call button from my grandmother and closed the door so she does not bother her. When my grandmother did physical therapy a cleaning staff member (lady) made fun of my grandmother saying she was pregnant because of her stomach when she has a tumor.

My father was in Avantara for five days, the staff was very attentive we especially appreciated Dr. Mandahmi , Nurse Dave and CNA Andrea, their Wound Care team was excellent!!!!!! I think the food could’ve been better, but they do provide a refrigerator in the room so you can bring in some food They are doing some remodeling now, which will help the whole place Look nicer and more updated.!!!! If my dad needs rehab again, we will choose Avantara

We had a terrible experience with Kristen, the social worker. Our family member was at the Libertyville location. When it came time to find a skilled nursing home, Kristen told us the only facility that would except our family member was their facility in Long Grove. We said we wanted a closer location so that we could visit our family member often. Kristen knew that we would not be able to bring our family member home because round the clock care was needed, and the only people who were at home were our extremely ill,elderly parents.( Our mother was dying from cancer.) Our family was under extreme stress just trying to take care of our parents. So she used a scare tactic. She said either we send him to their Long Grove location, or the facility was going to transport him straight home immediately, which I already mentioned was not possible. Kristen was well aware of this. Our friend, recommended a facility that was only 10 minutes away from the family members. Now that I have some real experience dealing with hospitals and nursing facilities, I know that patients do have some rights,and she really wouldn't be able to send a patient home if there was nobody to take care of them. What really angers me is that I was always under the assumption that a social worker would be somebody that actually cares about people and strives to help patients. I was proven wrong. I waited a long time to write this review because I wanted to give myself a chance to calm down. I thought maybe this anger would pass, but I just can't forget the treatment my family received.

I want to take a moment to express my heartfelt gratitude to all of the CNAs, nurses, physical therapists, occupational therapists, and staff overall at Avantara Libertyville. My dad was there for long-term care in skilled nursing for about 2 months before he moved into independent supportive living. He spent this time in the 2 South wing, where he received exceptional care. Audrey stood out as exceptionally great support for my father during this time. She went above and beyond to get to know him, and all of us when we visited. She looked out for him and helped him feel real comfort and care The business director, Kadri, was also excellent at being proactive and helping us navigate a big transition with patience and understanding. Everyone we interacted with during this time has been stellar and we really appreciate it.
