We had the pleasure of taking a wonderful tour of Avalon today and not only were the residents happy and engaged, the renovations were beautiful. I especially like the all female memory unit where you can tell the love and care put into those residents’ lives is a bar set high. I would highly recommend this facility to anyone looking for a caring and clean environment!

Avalon Rehab Hamilton is truly an exceptional facility. The level of care, compassion, and dedication shown by the staff is remarkable. Every team member genuinely cares about the residents and goes above and beyond to ensure they feel comfortable, supported, and valued each day. What stands out most is the passion the staff brings to their work. From nursing and therapy to housekeeping, dietary services, activities and administration, everyone works together as one team with the residents happiness as their top priority. The positive atmosphere, strong sense of community, and commitment to providing quality care are evident throughout the facility. My mother was transferred to Avalon Rehab Center on May 1st due to fractures and will be discharged soon after a 10-day stay. I would like to take this opportunity to sincerely thank the entire staff for their excellent care, kindness, and professionalism throughout her recovery journey. A special thanks to the wonderful nurses, especially Lolita and Patel, as well as the CNAs, including Candy and the gentleman who assisted during the night hours. We are also deeply grateful to the physical therapy team — Desai, Norma, and Carrine — for their dedication and encouragement. Thank you as well to Patti (Social Worker), Nasir (Scheduler), Yasmin (Activity Director), Ummah Shah, Kristina (management), the front desk staff, food service workers, and the cleaning staff for all your support and care. Because of the teamwork and sincere efforts of everyone at Avalon Rehab Center, my mother is doing much better with her mobility and daily self-care activities. Thank you all for the outstanding work you do every day. Please keep up the excellent service and compassionate care!

DO NOT STAY HERE! I would give negative dtars if I could! I transferred to "Avalon Rehab and Healthcare Center" under the guise that this place focuses on subacute rehab but once I got there, I realized that it was a really bad nursing home operating under the new name Avalon, Hamilton Continuing Care. I felt comfortable believing the google reviews and all of the marketing on their website because i stayed at an acute care facility under the same parent company. But once I got there I learned they in fact DO NOT prioritize rehabilitation and only offer MAYBE 20min of therapy a day to patients. Being there for 3 weeks, I received approximately 2.5hrs of actual therapy. On top of this, the conditions of the facility are below subpar and not even ADA compliant. I am non- ambulatory and my wheelchair did not fit in the Jack and Jill bathroom meant to be shared by 4 people, or the room meant to be shared by 2 people. The hallways are constantly cluttered with equipment (see attached photos). They are completely understaffed as well. So much so they rely heavily on per diem employees. There was a weekend that was so bad that there wasn't any CNA on the shift for overnight just a training nurse and a supervisor for the wing. A whole shift can go by without a CNA checking in. Average call bell time is 30min but I've waited up to an hour. The staff is not trained to do board transfers only hoyer.The nurses handing out medication late and sometimes the wrong dose or to the wrong patient entirely. Housekeeping is a joke. The floors are only swept and mopped only by request, biohazards like urine waste bags left on floor for days full. For breakfast 5 days out of the week they serve powered eggs and soggy toast. One morning my plate looked partially eaten and the staff tried to tell me it was the way the bread was cut but swiftly got me a new tray. (See photos of both trays) All of this while outwardly adverting themselves as Avalon but conducting business under Hamilton Continuing Care. For my entire 3 weeks there I did not see one document or medical request that used the public operating name. All these bad things are still being recorded under the old name so when you look up the new name it is rated a 4.6 on Google and I can tell now that the reviews are paid for. Many of the 5 star reviews are by staff (I recognize the names) or vendors & contractors doing business with the facility. Some are just random people, some posting multiple times, I can tell because one of them from a month ago specfically says they play bingo there frequently BUT they don't even offer bingo as an activity and I know because I've seen the activity lists for both February and March and SHOCKING NEWS, no bingo any of the days. This place is not for anyone under 65 with their mental facilities. PLEASE do not make the same mistake I did. I now understand why they always have openings for admission where the REAL rehab locations do not. While there I did make several complaints internally about the conditions and treatment as well as externally to the corporate hotline. Also, I am actually posting photos since no one else seemed brave enough to do so. Not sure how people are calling this place bright and clean

My mother has been in the long term care area going on three years. She has been in 3 different wards. My family and I have found the nurses and staff to be very attentive and caring. Some of the stand out nurses are: Dan and Maria (aide) both very patient with my mom, Susan, Ginny, Shelly (supervisor), Candy and Leslie (a former nurse who occasionally fills in for the other nurses). The receptionists are outstanding both Dorothy (week days) and Mary (weekends). After checking other facilities, this was definitely the right fit for my mom. My uncle and a former neighbor were also there and they were both well cared for. If you want a place that tries to make as close to “home” for your loved one, this is the place. They also have activities for the residents each day to keep them active! My family and I highly suggest checking it out.
