On July 30, 2021, I placed my husband in Carrollton Autumn Leaves memory care facility. He was rushed to the ER twice the first week he was there. One reason Autumn Leaves was chosen is they said they had hall and common area monitoring. He was a Vietnam veteran and they said there were two other vets there as well. Tuesday morning around 1:30 am, he had an "unwitnessed" fall they said was not picked up on the monitor. The following Friday around 4 pm, I got a call to let me know he had fallen again but from a dining room chair and was being taken to the same ER. After being released that time, I took him home and the next day I went over to Autumn Leaves, packed his belongings and returned the key. I sent Lucy, their executive director, an email documenting the reasons why and told her I no longer felt they could safely take care of him and asked for the return of the remaining room and board I had paid. She never answered any of the subsequent emails or calls from me so I filed a civil suit for the unused portion of R&B + Community Funds and won the judgment (Sept 1, 2023). As of this day (July 23, 2025), they have refused to acknowledge or even answer any of the post judgment appeals I have sent. My experience Autumn Leaves and the owners, J&M Family Management, has been terrible and very disappointing. Mrs. J. My mother has been in Autumn Leaves assisted-living for about one year. The experience that she’s had has been really good. We are satisfied with her room and the facilities, and very satisfied with the caregivers who tend to her regularly. I would not hesitate to recommend their assisted living facility.

My Dad had emergency surgery & needed to be released to a skilled nursing facility and Autumn Leaves was on the list. Since it got good reviews, my family & I wanted to tour the facility. The care coordinator took me, my mother & my brother to a nice & clean floor, so we decided to have my father sent there. However, but my Dad was on a totally different floor. His room and the furniture were filthy and stank, he was covered with ant bites, the dining room was messy & the floor was sticky, so of course, we had him transferred to a clean, nice and pest-free facility. The aides & nurses do what they can, but Autumn Leaves is seriously under-staffed. Fortunately, my father has family who can advocate for him, but I feel badly for the Autumn Leaves residents who don’t. I will NEVER recommend this place to anyone.

My dad has been a resident of Autumn Leaves for over a year now. It would have been difficult for me to write a good review 6 months ago but now I couldn't be happier with the new staff. Josh has been a godsend and he is working hard to bring Autumn Leaves standards to a respectable level and he is truly achieving his goal. The new staff takes the time to get to know their residents and adapt to their different needs and wants, there is a lot of warmth keeping it professional and respectful. The staff is very well trained and they know what they are doing. The rooms are spacious and the options of different levels of care is very convenient for the changing needs of their residents. I recommend Autumn Leaves, I am very pleased with the place.

I have worked with Autumn Leaves for the care to my parents. They have been very responsive to any issues presented. I am grateful to the personnel at Autumn Leaves such as Debi, Josh, Dee and Angela for their help. Personnel at Autumn Leaves is experienced. Autumn Leaves has many levels of care. This is highly desirable for the changing needs of their clients. The ability to have independent living to nursing-rehab is very important. This facility is close to many hospitals. I am pleased with the care my parents are receiving.
