I visited my friend and she is doing great. If I need post op care this is the place I would choose. They have excellent care. It's like being at a resort. Her knee surgery is healing fast and she is walking now. She loves everyone here. Great physical therapy, occupational therapy, housekeeping and nursing are top notch. 5 stars.

Autumn Creek Rehabilitation has been a great resource for several of our patients needing 24-hour nursing care and short-term rehabilitation to safely return home with our home health. I was happy to see and hear that Johnny has been assisting with patient placements here, as well as with their two local sister buildings. As he is efficient, knowledgeable, and experienced in the healthcare field. I have worked with Johnny for the past 12 years in healthcare and can confidently say he is always responsive and available when our agency reaches out. We truly appreciate the recent assistance in accepting one of our residents who needed additional in-facility rehab!

Autumn Creek Post Acute is a wonderful facility that truly stands out for the quality of care they provide. The building itself is clean, modern, and very well maintained, creating a comfortable environment for patients and families alike. From the moment you walk in, the staff are warm, welcoming, and clearly dedicated to making everyone feel supported and cared for. This facility specializes in short-term rehabilitation and has strong outcomes for patient care. It’s reassuring to know that they offer 24-hour nursing staff, ensuring that patients receive attentive, skilled care around the clock. The team works hard to help individuals recover and regain their independence in a supportive setting. A special thank you to Kandi in Social Services, who is incredibly caring and always quick to respond. She goes out of her way to help families navigate the process and makes sure everyone feels informed and supported. Johnny also deserves recognition for the way he goes above and beyond for both his staff and patients. His leadership and dedication are clearly reflected in the positive atmosphere throughout the facility. If you are looking for a short-term rehab center that provides compassionate care, excellent communication, and a clean, modern environment, Autumn Creek Post Acute is a fantastic choice.

During a time that can be stressful and emotional, the staff here have consistently shown professionalism, kindness, and genuine compassion. I especially want to give a heartfelt shout-out to Makala Conn, my grandpa’s CNA. Makala is an exceptional caregiver and truly stands out as an employee. She is attentive, patient, compassionate, and always goes above and beyond to ensure my grandpa is comfortable, respected, and well cared for. Her kindness and dedication make a meaningful difference, and it is clear she genuinely cares about her patients. Because of caregivers like Makala, our family feels confident and at ease knowing our grandpa is in good hands. Autumn Post-Acute is fortunate to have her on their team, and we are truly thankful for the care provided.
