Patient was here from August to January 2026. Giving this an 1 star is far from what it should be. Our family member was given an intake and the doctor deemed him incompetent.(family did now this) The staff knew that his wife was his POA in all medical decisions with proper paperwork. The doctor there never spoke too/saw his wife at all when he was present. Without even speaking with the wife or the social worker he deemed the patient incompetent and called SSA to let them know he needed a payee. Which was confusing why the case worker did not tell the doctor his has an POA which is his wife. The doctor said he will not get any better and needs long term so no more PT for the patient. The social worker (Rose)does not return any calls,or keep family updated. Patient wife is there 6 days a wk and never can talk to the social worker. The social worker told the wife on a Friday that his discharge date was that Monday. Family could not do anything due to the wife was told actually at 4:55pm. The wife had to wait til Monday to find out what's going on. Then the corporate called and told the wife he was to be discharged due to Medicare stop paying and was being transferred. The wife told corporate that she was appealing it since she feels he was not ready to be released . After that the care got worse. The family felt the lack of care got worse once they filed the appeal. The social worker told the wife 2 times prior her leaving for the day he is being transfer. The patient wife told the social worker Rose no he is not and by law they can not because they have meeting for the appeal. The appeal was denied, the social worker Rose called the wife 1 1/2 hr prior to let her know it was denied and patient was being transferred at 2pm. The wife said no because she was not notified from Medicare and by law she has be notified. The social worker Rose told the wife if she denies his transfer she was calling the police on them. The wife inform Rose that she could do that but the police would tell her it is a civil matter and she has POA so its her choice to deny it. The wife inform Rose legally by law he has 24hrs to leave from notication of the appeal denial, and to not touch his belonging since she will be there to get them. Rose said okay and hung up on the wife. When The wife and family showed up patient belongs were packed all in plastic bags like he was not a human being and thrown on his bed. Corporate showed up to make sure his stuff was packed in boxes which they were not. The wife showed him and told him what she told the social worker Rose and he pulled Rose into a room with the head nurse. During the Medicare appeal meeting the patient wife asked how can this doctor make this decision if he only saw my husband once,why has he never talked to me to ask questions about his medicine,daily routine prior coming in and etc.. The doctor stated in the appeal meeting he did not have to talk to the family but, he has seen her there with the patient. Why the social worker did not step in and inform the doctor that he has too due to she is his POA. Family feels that is unacceptable! This facility changes medicine or stop it without letting them know. Also,the family is questioning some info on his charts. Patients was hospitalize for covid for wk and when family got his chart it shows that whole week of his blood sugar reading. How is that if he was hospitalized for covid. When calling to ask questions the family just got no call backs. The same doctor at Almaden is the same one that goes to White Blossom nursing facility. I would be aware of this when looking into facilities. Corporate guy that was there said, he knows that the doctor does not talk to the family and they are working on it. So corporate is very aware of how their doctor is doing stuff. Below is pictures of how they packed the patient belongs. Yes, the facility is clean but the staff is lazy. Make sure mark everything that comes in since stuff was stolen and we still never got or etc....

My Brother had a Stroke that left him with Serious Paralysis on his leftside. Wheelchair is his mobility. After Going to Almaden Health & Rehabilitation, His Quality of life has Improved significantly! He is able to use a Cane and Walk without always needing his Wheelchair The Physical Therapy Dept. Is Top Notch! They are Patient and Knowledgeable. This is Key for Successful P.T. Achievement. Also, The Healthcare Staff that manages the patients, healthcare needs are well organized and Know there jobs well. Fresh Leadership isn't 100 % the Fix. However, in this fresh leadership, It is 100% a strong reason for the Improvements. Social Work and Case Management, Managed By Rose who is Very experiences and Knowledgeable Discharge Planning as well as Helping Patients and their families Navigate Complex Areas Such as Medicare Coverage and all the Confusion that Tv Commercials have caused. Rose was Insturmental in helping my Brother choose the best coverage nased on his Specifice Healthcare needs. Thank You Rose! Nursing Home A.D. Newud is. Sharp Young Man that Is making Solid Decisions for the Facilities Overall Improvements. He Thinks like a Leader and Speaks as an equal Staff Member. Creating a positive Workplace. I'm surprised this facility hasn't had any law suits. The nursing staff is very careless and there are medication errors all the time. If the resident doesn't know their medication then I wouldn't be surprised if they weren't getting the correct amount or correct medication. The Good LVN's they've had just end up quitting because they can no longer put up with the Director of Nursing. Most of the CNA's are good. There are some who'd rather be on their phones than answer a call light. The food is HORRIBLE! All of the kitchen staff is unable to read the tags for the trays. You end up getting food your not supposed to get. A lot of the food is expired and served several days in a row just in different ways. The showers have black mold inside and need to be fixed. I wouldn't recommend sending any family member here!

My mother-in-law stayed in this facility for almost 5 weeks for rehabilitation. The entire staff (nursing, therapy and CNA) were very courteous to my mother-in-law who is a non-native English speaker. They worked with her very patiently to ensure that she ate her meals, took her medications and regain her physical strength to sit, stand and walk using a walker. Overall we were very happy with the services provided by the staff at this facility and it enabled her to come home.

On Valentine's Day, 2/14/2025 (Friday), I called Almaden Health & Rehabilitation because my sister was not answering her phone. I live out of state and wanted to wish her a Happy Valentine's Day. I called the facility main number three times. The first and second times, I was placed on hold. I may have been forgotten. I called a third time, and the call was answered. I left my phone number for a call back with an update regarding my sister wellbeing. Saturday, 3:51 p.m. EST, I am still waiting. All I wanted was a simple update regarding my sister. I understand nurses work long hours, and I appreciate the work they do. I am disappointed with the lack of communication. Thank you for reading my review. Best regards.
