Worst experience ever. I would not recommend this place for anyone. My mother was transferred there for rehab after being discharged from the hospital for rehab. She also has dementia. The therapy department was absolutely wonderful. Very patient and professional while working with her. Also very caring to her needs. As far as the nursing staff and cleanliness, very very disappointed. Nursing staff never would come and check on her. They would come for her medication time. Hand it to her and walk away. Would not even make sure she took it. Family would come and find a pill here or there laying in her bed. If her button was pushed for the nurse by one of us so that she can be changed or if she needed anything, it would be hours if they were to show up or we would have to track someone down for help would still be a while for any kind of help. We would actually have to change her own bedding most of the time, mind you the bedding was disgustingly stained. Her room was not very clean and the floors were alway sticky. THE WORST EXPERIENCE we had was one morning the CNA was helping my mother get dressed and while getting her dressed she fell. The CNA told my sister what had happened as soon as my sister got there. She told her that she fell while helping her get dressed and that she hit her face on the side of the nightstand and then she lowered her to the floor. My mother did nothing but cry and complain from being in so much pain all on the left side of her body. My sister demanded that she be sent to the hospital for X-rays . When myself and wife arrived later that day, we asked the nurse what had happened and she blew it off and said the CNA was wrong and none of that actually happened and that my mother just turned over on the bed and hit her face on the nightstand and that my sister made up the story per the CNA and the nurse. My mother did actually have a fall with the CNA. She did in fact fracture her hip. We have the X-rays and pictures to prove it. Also she is still in the hospital to this day and the fall happened exactly a week ago.

This is such a warm and inviting property. My Mother is so happy here and has made so many new friends. The staff has been amazing. Our family couldn't be more happy that we were able to get her moved in. She is living her best life here.

I'm here for rehab on the skilled nursing side and it was awesome. The staff was marvelous, the food was great, I really enjoyed therapy the staff attitude were beautiful. And the patient advocate just awesome. I would recommend anyone who needs therapy to come here. I definitely met my therapy goals

I’m here for therapy on the snf side and therapy is good. I just feel that they need more staff. But most of the staff that is here is good. My social worker Angelic is Great, Patient advocate is friendly and nurse Destiny is my Favorite. I will recommend this place to anyone who needs therapy

I am writing to formally express my serious concerns regarding the care my father is receiving. His room is consistently dirty and has not been properly cleaned in approximately two weeks. When he presses the call button for assistance, no one comes, leaving him waiting for help for extended periods of time. This is deeply concerning and unacceptable. In addition, several nurses have been rude and dismissive, and aides frequently ignore his requests for help. My father deserves to be treated with dignity, respect, and basic standards of care, and at this time, those standards are not being met. I am requesting that these issues be addressed immediately. I would appreciate a prompt response outlining how this situation will be corrected to ensure my father’s safety, cleanliness, and well-being going forward. Thank you for your immediate attention to this matter.
