Rehabilitation and Nursing: Elevate confidence and ability

Rehabilitation and nursing care lessen the trauma resulting from an injury or physical disability with 24/7 dedicated care by a diverse team of specialists. Residents are provided elevated care with a personalized wellness plan that is designed to restore confidence, ability, and independence.

Post-Acute Rehabilitation

In recovering from an injury or illness physically and emotionally, our medical team provides a unique pathway for each resident or outpatient for post-hospital care. Our experience extends to individuals following a traumatic injury or major surgery.

Orthopedic Rehabilitation

Our diverse interdisciplinary team will collaborate on an individualized treatment plan for individuals post orthopedic surgery or major bone injury. Our experience includes individuals with a hip or spinal fracture as well as total knee or hip replacement.

Stroke Rehabilitation

The challenge of regaining critical skills and physical abilities lost after a stroke is made easier with our expert licensed therapists who develop a focused rehabilitation plan for each stroke survivor. Collaborating with the resident and family, our team creates a customized plan with suitable therapies that stimulate lost physical and emotional abilities.

Cardiac Rehabilitation

Our program includes providing education on one's specific cardiovascular disease while learning and improving exercise tolerance to open the road to a healthy lifestyle and strong heart. Our experience with individuals sporting congestive heart failure, coronary artery disease, coronary artery bypass surgery, ventricular assist devices, heart attack and other cardiac malfunctions.

Hospice care: Elevate the quality of life

When the road to recovery is challenged, our chronically ill residents receive specialized end-of-life care with physicians, nurses, social workers, and clergy providing complete support to elevate each resident's quality of life and live each day as fully as possible. A devoted team of diverse specialists establishes a committed relationship with the resident and family, supporting the resident's personal goals each moment.

Respite and short-term care: Elevate your expectations

In our welcoming community, a new face is refreshing to staff and other residents. Respite and short-term care provides the unique experience of unwavering, dedicated care like a long-term resident. Our friendly and dedicated staff work to ensure all short-term residents receive the appropriate level of care needed. Our goal is that our temporary residents leave cared-for and awaiting their next visit.
