I would like to give them a zero rating but the rating starts at 1 .. so I will give them the 1 star and that is only because tbe floors look nice and waxed !! No nursing home is a great nursing home however This guardian health at Windsor on Belmont Avenue is by far the WORST nursing home I’ve ever seen!!! It is literally a death trap !!!!!!!!! If you want to INCREASE the likelihood or chances of your family member dying then this is the PERFECT facility 100% !!!!!!!… My father was there for two days and fell two times !!!! Unwitnessed fall of course which is usually the case in these facilities because nurses DO NOT routinely check on patients especially at night !!! So no telling how long he had been on the floor before they found him at 630am! When they heard him say “ hello”??? And he’s on blood thinners and the nurse did NOT think that it might possibly be a good idea to send him to ER!! ?????? He was supposed to have a CPAP machine so that he could use every night to prevent a CO2 buildup And for both days they have no CPAP machine for him to use So his Oxygen saturation level was below the normal range because his CO2 had built up … from not having his CPAP Machine for any of the two nights that he was there !!!! So when the night nurse calls me before she leaves her shift in the morning .. calls me at 830am to tell me that “your dad fell but he’s ok he was a little confused didn’t know why he got up out of bed but his oxygen is at 91” ( which she as a nurse should surely know is NOT a good number as she says it as if that’s a normal number and nothing to worry about) and she should have known that he was on blood thinners from the information she received from the hospital handoff So yeah nurse if yall did not give him his CPAP for two nights.. his CO2 will build up and cause confusion!! Something else a nurse should know !!! And nursing 101…. A nurse should know if a unwitnessed fall in an elderly patient on BLOOD THINNERs .. and the nurse says “ and he doesn’t know if he hit his head “ Exactly nurse i!!!! That is another reason why you send the patient straight to the ER!!!!! Also nursing 101…. teaches that in an unwitnessed fall you always have to assume they may have hit their had !!! So imaging needs done asap!!!! And even more so if the patient is on BLOOD THINNERs!!!! So when I arrive to the facility we get him to the ER and it is here we find out that. His CO2) was 92%!!!!!!!Almost 3x the normal range so for the nurse to say oh his Oxygen “Saturation is 91 but he’s ok” that’s not a competent nurse !! Ya think that may be the reason why the pt was confused !! ?? Brilliant nurse…. He was in bad shape when he got to the ER!!!!! D/t this facilities lazy and clearly incompetent staff This ineffective and totally incompetent staff Will KILL (not exaggerating) your family member if you do not know your stuff .. there’s no telling how many preventable deaths have occurred there !! If you love your family member and want to ensure that their life span is not compromised by incompetent nurses then DO NOT BRING THEM TO THIS Facility I swear you will regret it !!!!!!!! Worst facility in Youngstown!!!?!!!!

They tell you you’re coming for 30 days therapy but try to keep you as a permanent resident against your will instead of giving you therapy. No one will talk to you about discharge but will continue to submit payment. I want out.

Windsor guardian health care in Girard was providing the level of care needed for my brother in law after a recent hospitalization . The nurse’s aides housekeepers security interacted with residents with a helpful and courteous demeanor . We had a particularly hard week and this trip from Columbia to see Sam was complicated with heavy snow and terrible driving conditions . We were really shown every hospitality while helping him order a new phone and get messages to his his nurse and therapist on Monday . Keep up the good work Guardian !!

It's a great place gives you the help you need and respect you deserve as a person. A great atmosphere for family of all ages, must say the food isn't my cooking but it's good portions could be bigger but a closed mouth don't get fed. Just ask and they will provide. I must say I heard alot of negative things about this place. But from my experience it was more lies than truth. Yes some aids bring their home problems to work but you can't let one or two bad apples ruin your apple pie. I give Guardian a thumbs up and would recommend them to anyone in my family or anyone in need of a great place for care and support...

Poor and Sloppy Care As many family members/spouses have recommended if you love your loved one DO NOT PLACE THEM IN HERE!! My husband was not getting the PT/OT/and was not treated properly for the wound on his butt....it is now twice the size it origninally was, bigger thanks to them. That is why the PT was so important so he would not just sit in the bed which is what happened. He needed to be rotated every two hours. Because I filed a complaint with my husband's insurance I received HELL and my husband was DENIED HIS MEDICINE. IF A PERSON IS THERE THE SPOUSE OR FAMILY MEMBER, THEY ARE NOT ALLOWED TO FILE A COMPLAINT. NOT GETTING THE SERVICE FILE A COMPLAINT. The other day when I went to see him I asked what's up? Are you okay? he said NO. I asked why didn't you call someone, he said he did but no one came to help him. Walker across the room he had to get, get to the bathroom and he had an accident urine on the floor, & diarrhea on him and I cleaned him up. We tried to clean it up in the bathroom but I had to ask ftwice for help. We couldn't find a gown but again after ringing buzzer no one came so I went to ask for a new gown. Of course, no one their wears ID badges so you do not know the person's name. So if you go to this agency read reviews by other folks. Always read the reviews. If I could give ZERO I WOULD I would. Glad to have my sweetheart at home and I am happy to serve him which their service is very LOUSY. My husband is always grateful for what I do and he let me know that ALL THE TIME.. I will always be a patient ADVOCATE. If they don't want complaints then change things for the better; do your job and stop playing games on your phone which I noticed while I was there. I have been called a liar when my husband asked me to write a letter stating the day he wanted to leave. A person stood in the room and told me my husband did not tell me that when she was not there. The conversation did take place with my husband and he did ask me to write it. Also had a comment thrown at me which was: why did you let him have his wallet and keys since he has dementia?? Don't leave your personal property there; somehow it grows legs and walks. People do not leave your love ones at this establishment.
