Upland rehabilitation shout out to your activity department they are the best they do an incredible job at providing a comforting and fun environment. My grandma always looks forward to the daily activities, whether it's bingo, live music, or painting with memo. The activity staff is incredibly patient and enthusiastic, which really brings the residents’ spirits up. A huge shout out to Amy Moran, Alondra Cortez, for being very attentive to my grandma and always making us laugh and smile give these girls an increase of pay please.

If you hate your loved one and are praying on their downfall, then Upland Rehab is the place to send them! I am in absolute disbelief that this facility is still operational and hasn’t been shut down by the health department. The moment I walked in, I was hit with a strong, foul odor—it smelled like a gas station restroom, and I gagged. The entire place felt unsanitary. Staff were spraying bleach directly into patient rooms, seemingly to mask the smell, and the chemical exposure was so strong that my eyes burned for days after I left. After just two hours inside, I felt physically unwell. We brought my dad food, and I couldn’t even eat it because of how overwhelmingly disgusting the environment was. I have never seen a rehab facility in this condition. The building itself appears severely neglected and poorly maintained —Like it survived a zombie apocalypse. My dad was transferred here after a short stay at San Antonio Hospital, and within three days, I did everything I could to get him out. During his time there, a nurse attempted to administer medication he was not supposed to receive. Another staff member entered his room in the middle of the night and stood there staring at him like a weirdo, which was deeply unsettling. He had blood in his urine that went unaddressed, and there were long periods where he was not properly attended to or changed. The food was also extremely poor in quality. Don’t get me started on the admin side of things. Jeff who is running the place is beyond incompetent and I believe he is largely the reason why this place is so awful. I was trying to get my dad out and Jeff was doing everything he could to make the process a living hell. He was beyond disrespectful to me, put words in my mouth and was incredibly unprofessional. I can’t believe someone who is that bad at their job is employed, but I guess it makes sense for the most horrific “rehab” facility in SoCal! I would get this place -10 stars if I could.

My mother spent 34 days at Upland Rehabilitation & Care Center following a stroke, and the experience was defined by the responsiveness and diligence of the staff. One of the most notable aspects of the care was the engagement at the nurses' station; staff members consistently made eye contact and offered assistance immediately. Even when a request fell outside their specific role, they would often personally walk me to the correct person rather than just pointing the way. I also want to mention the physical environment. While the facility is older, the management team, led by Enrique, maintains it with clear pride. I had the occasion to be on the floor retrieving a dropped item and was struck by how clean the space was, even in hard-to-reach corners. Finally, the support from Jacqueline, the case manager, was instrumental. She became my primary contact for navigating my mother's needs, providing reliable answers and ensuring things were handled promptly throughout those weeks. Knowing my mother was in such a secure and well-maintained environment made a difficult situation much more manageable.

If my grandma was never placed in this facility she would still be alive today I promise you this! First and foremost, if you truly care about a loved one, I strongly advise against placing them in this facility. The level of care provided by the nursing staff was deeply concerning and, in my experience, unacceptable. My grandmother passed away while under their care. During my visit from 1:00 p.m. to 9:00 p.m., staff were supposed to reposition her every two hours, yet this was done only once during that entire time. This lack of attention was alarming. My grandmother had difficulty eating and was prone to choking, frequently asking for water. Another patient in the room informed my family that after feeding my grandmother, staff did not return to check on her, which caused her to choke. This neglect very likely contributed to her passing. The facility failed my grandmother. Additionally, the nurses were consistently rude and dismissive. The patient sharing the room with my grandmother did not speak English. Rather than taking appropriate steps to communicate—such as contacting her son when there was a language barrier—the staff repeatedly stated they could not understand her and made no effort to assist. Ironically, many staff members spoke limited English themselves, yet no accommodations were made for patients. This same patient told my family that nurses did not respond when my grandmother repeatedly asked for water. No one came to help her. This facility must be held accountable for its actions. Patients deserve compassionate, attentive care. If staff are unhappy in their roles, they should seek employment elsewhere rather than allowing their lack of concern to impact vulnerable individuals. Lastly they did not let me aunt know that my grandmother passed away. They let her walk into the facility into the room where she was laying there and then told her that she passed smh. This is the worst facility in California. I hope Every nurse that was on my grandmothers shift ends up in jail.

My mom was a patient at this facility; discharged 11/6/2025. THANK YOU to the entire staff, the skilled nursing team, the CNAs… literally EVERYONE treated my mom with the utmost care and kindness. Jamima, Alyssa, Noah, Nancy, Thiri, Joani (I know I’m forgetting a few more), but these people were directly involved with my moms care: THANK YOU🤍 I never thought my mom would be in a position to have to be in short-term care outside my home, and to say I was worried and stressed, would be an understatement. But after the first week, after seeing how well my mom was being cared for, every doubt I had went away. Thank you all so so so much, Upland Rehab!! You all are the real MVP’s💯
