Fantastic facility, great staff. They’ve been taking excellent care of my father there. The nurses, aides and therapists are wonderful, and really get to know each patient to give them a personalized path to recovery. I want to make special mention of Robyn Maimone, the Director of Hospitality. She has been so helpful & wonderful ever since we first arrived! From the very beginning, she has always gone out of her way to check on us, and make sure we have everything we need, and that any concerns we had were tended to. She always greets my father with a smile and a sunny disposition, and makes him smile as well (which is not always easy!). Every interaction with her provides me with so much useful information! Senior care can be overwhelming & confusing, but with people like Robyn, who really care, it is so much less so! Great note of appreciation to Robyn and The Subacute in general! I’m glad we chose this rehab!

There are a few things that need to be looked into. My husband had been here 4 weeks and has had 3 showers. And they happen after we ask constantly. The food is not good at all. Nursing staff are good and there are some really good aides. I have seen a few aides who do not go out of there way to help. We were told therapy would be 2 times a day and this has only happened a few times. Therapy ordered a knee brace for my husband and 2 weeks later we are still waiting for it. Disappointed that we are not given a weekly update on progress of my husband.

The Subacute at Autumn Lake is thus far a positive experience for my husband. The Staff we have interacted with are friendly,courteous,professional,inclusive and supportive. PT/OT services are receptive to pain management and notify the Nurses. Nursing staff were supportive and listened to my concerns about my husband’s change in condition and he received necessary treatment in a timely manner. Activities are fun and varied and everyone is welcome to participate - everyone seems to enjoy themselves and have a sense of community. Independence, positive self esteem are encouraged and you are treated in a dignified manner. No Health Care Facility is perfect but Autumn Lake puts forth the effort to help you have a positive experience. It is clean. If you have questions or concerns the Advocate addresses your issues in a timely manner. They try to meet your dietary needs - it’s not perfect but they try. My husband was feeling defeated and hopeless about PT/OT that his status wouldn’t improve ( because he was told that in the past) - they encourage and work with you to achieve your goals and stress safety to ensure that you can return home to your family. Most important they listen and want to improve their services.

Mother’s Day was absolutely beautiful. From the flowers to the smiles to the constant updates in our family group chat, we all kept saying the same thing… Mom is being so well taken car of. Michele and Robyn from the Hospitality Department truly went above and beyond. They delivered flowers, checked in constantly, and made us feel like family instead of visitors. There was such a warm and comforting feeling throughout the entire day that honestly made it feel like home. Every single person there has been kind, attentive, and geniune. It means more than words can explain knowing your loved one is surrounded by people who truly care. Thank you to everyone for making Mother’s Day so special for our family

Thank you so much for the care you provided to my mom during her stay. I’d like to extend a special thank you to Quiana and May from Activities for always making sure she was able to participate. I’m also grateful to the clinical team, especially Richard (Night Nurse), and Simone, your Physicians Assistant for being so attentive to her medical needs and making necessary adjustments to keep her comfortable. A sincere thank you as well to Robyn and Michele from the Hospitality team for ensuring she was always comfortable.
