Very clean facility... I've been visiting my client for the past 3 1/2 weeks. Friendly staff from the front desk receptionist to the Nurse, CNA, OT, Food Servers, Janitor, Maintenance and Case Workers. Nurse Appreciation Week was displayed perfectly!!! Definitely were a lot of smiles 😊

I cannot say enough good things about the staff especially the PT experts there including but not limited to Clarissa, Marcus, Julio, and several other people.

They give medicine late, more than 3 hours. This is not the 1st time happened. If your loveones dont have someone to look if the medicine is compromise not a good place. The patient might gonna die early if no one will monitor.

The name has changed to Bellefontaine Health Care Center or at least it was during my recent stay. I chose this place as I was leaving Huntington Hospital after my trauma accident. I was in this hospital Oct 27-Oct29. I was taken to Bellefontaine and although I returned to the Hospital for 3 more days I was there until December 13th. The main reason I picked this place is the 2 hours of therapy I knew I would need and I am very glad I made this choice because Matthew and his crew are very caring and helpful getting people to overcome problems. They did not disappoint me and I have raved about them since leaving. Everyone was friendly and I enjoyed meeting them all. I would have liked to meet the doctor but he set up good appointments for me with the plastic surgeon (face), orthopedic doctor (broken left wrist) and also my neurologist (change made to my usual medicine) during my stay. The nursing staff was very attentive and how they balance the meals, showers, changes etc is amazing and with a smile at that. I also want to say thanks to Myra (from Activities) who accompanied me to my doctor visits and made me feel relaxed and the Receptionist who helped me make out all my paperwork as my right shoulder needed help also. As you can see I believe in patience, treating others as you would like and I can still remember many of their names. I did not go to the activity room because I found the library and I could get lost in books in my spare time. My roommate was a 97 year old lady who I really adored. I hope I have given you the information a man names Charles has called me about since my return to my own home in February. Thank You to all the people I have had contact with and the help I received. I have not been in Hospitals except on an outpatient nor have I been in a rehab facility but both of you get my high approval.

My mother died in this facility. She actually fell and broke her hip because they ignored the calls for help for assistance going to the restroom. She was a difficult patient which I understand, however if you continue giving opiate medication to a feeble patient until they go into respiratory failure, shame on you!!!!! Her bed was disgusting without sheets, only a blanket. It was horrible to watch. Please be careful if your loved one has a DNR order. As they will be more than happy to get another patient that needs more service so they can leech off of. After all, that's how they make there money. The staff acted nice when I visited, but my mother said they ignored her calls constantly. When I asked for the medication schedule leading up to her death, I never got a call back or any answer. Don't send your loved ones to this place especially if they are a more difficult case.
