I visited spring Oak with a friend of mine whose grandmother moved into the community a few months ago. My friend was so excited to share with me her grandmother's satisfaction moving into this community, she's already made a few friends ,enjoys the activities and the friendly staff. I am definitely considering this community for my loved one.

Our family had several very concerning experiences with Spring Oaks while my mother was a resident, and I feel other families deserve to know about them before making such an important decision.<br>In September 2021, my mother returned to Spring Oaks after a colonoscopy. The nursing staff had her post-operative instructions, yet she was never brought dinner and was left alone in a recliner. She attempted to get up by herself, fell face-first into the corner of a dresser, and suffered extensive bruising to her face, knee, and toes. Even more heartbreaking, she later told us she had laid on the floor for a long time during the night, crying and screaming for help, but no one came. She wasn't discovered until around 6:00 the following morning. When I asked what their overnight safety checks consisted of, I was told staff simply walked the hallways and, if they didn't hear anyone calling out, they did not enter residents' rooms. Following this incident, we were told she would be checked every two hours. She was transported to Community Medical Center and then required approximately a month of rehabilitation at Hampton Ridge.<br>In January 2026, we received another devastating phone call. The Lacey Township Police found my mother wandering alone at Walmart in her pajamas after a Walmart employee called to report her. She had crossed Route 9 using her walker and had apparently been outside for at least an hour and a half. Spring Oaks later told us their last room check had been at 4:00 a.m. When I asked why no one responded when she left the building, I was told the front door alarm had sounded, but staff assumed it was an employee arriving for work and never checked. Had the police not found her, the outcome could have been tragic.<br>After that incident, she was moved to the locked memory care unit, where residents wear ankle bracelets that are supposed to trigger an alarm if they attempt to leave. However, on several occasions while I was visiting, my mother rode the elevator with me and no alarm sounded. Staff was made aware of these failures each time.<br>In March 2026, my mother called my brother late at night after falling in her room. She told him she had pressed her emergency pendant, and the green light came on, but it still took approximately nine minutes for staff to assist her. The following morning, management initially claimed no alert had been received in the previous 24 hours. When they tested the pendant, they discovered it was not communicating with the nursing station despite showing a green light. The entire emergency pendant had to be replaced. When I asked where my mother had fallen or for details from the incident report, the administrator could not answer basic questions and seemed unfamiliar with the report itself.<br>Right before we moved her to a new facility.<br>In March 2026, we also experienced a very troubling incident involving my mother's cell phone. One of the nursing assistants appeared to have swapped my mother's phone with a broken phone. When I arrived and realized something wasn't right, my sister-in-law and I completely searched my mother's room. We stripped the bed, removed all of the sheets, and thoroughly searched the entire room, but her phone was nowhere to be found. Then, the very next day, we were told her phone had been "found" in her bed sheets. That was simply not possible, as we had personally removed and searched every sheet the day before. The circumstances surrounding this incident left our family with serious concerns about what actually happened and further eroded our trust in the facility.<br>No assisted living facility is perfect, but families trust these facilities to keep vulnerable residents safe. Repeated falls, delayed responses, malfunctioning safety equipment, a resident wandering out of the building unnoticed, and poor communication left our family feeling that my mother's safety was not being prioritized. I hope our experience encourages improvements and helps other families ask the difficult questions before entrusting a loved one to this facility.

Moving my grandpa into Spring Oak was an emotional experience for our family, but the staff made the entire process so much easier. From the moment we walked in, everyone was so friendly, attentive, and reassuring. They took the time to answer our questions, made my grandpa feel comfortable and welcomed, and truly showed that they care about their residents. We're grateful for the kindness and support they provided during such a big transition. Thank you, Spring Oak!

I can’t say enough wonderful things about Spring Oak At Forked River Assisted Living. From the moment we walked in, we felt welcomed by the caring and compassionate staff. Everyone goes above and beyond to make residents feel safe, comfortable, and truly at home.<br><br>The facility is always clean, well-maintained, and has a warm, inviting atmosphere. The staff treat every resident with kindness, dignity, and respect, and it’s clear they genuinely care about their well-being. Communication with families is excellent, and it’s comforting to know our loved one is in such capable hands.<br><br>We are so grateful for everything this amazing team does each day. I would highly recommend Spring Oak Assisted Living to anyone looking for a place where their loved one will be treated like family.<br><br>Thank you!!!

My family and I toured this facility for a family member recently, and we’re excited to move forward! The place is so welcoming, the staff is always friendly, and it felt like home as we were walking around. It made it more comfortable knowing our family will be in good care
