So my father is 90 years old on hospice. While at Southgate, a therapist named Tyson was his primary therapist. She is a rockstar! When he was first admitted, he couldn’t any longer walk. By the time she was done with him, he was jogging through the halls! He also couldnt talk anymore, due to his health. but by the end of their time together, he was doing karaoke for everyone in the place!! I swear this woman has superpowers. I don’t know how she does it. I would give it 5 stars but the facilities are kinda gross. But Tyson if you are reading this. You saved my family and have restored my faith in humanity. Thank you for your service. But also tell your bosses to maybe clean up in there once in a while

Very caring staff and they all seem happy to be working there. Recent changes are evident in every aspect of this facility. The new owners are very involved and have already made many improvements. My mother and I are so happy with this facility and the care she receives. Keep up the great work!

Is it a pretty place? Not really, the building is a bit rundown but it is clean and the staff is caring and skilled. They took great care of my mother. I appreciate that they would address any concerns I had and kept me informed of any changes in care. I do recommend this facility

Horrible Place! Before we arrived with my father we were told that he would have a private room. He was placed in a room with a special needs person who was peeing on the floor when we arrived. No mopping of the floor it was wiped up with a towel. My father needed to use the bathroom and was left there unattended while the nurse helped another patient. After several minutes passed and still no nurse, I decided to check on him myself. I found him lying on the floor. He was trying to reach the toilet paper that was placed on the sink because the wall holder was broken. I immediately pulled the alarm cord near the toilet and ran into the hall to get the nurse. The nurse was no where to be found. I screamed for help and another nurse from a different hall heard me and helped. I then removed my father and found a different facility to care for him.
