Do not bring anyone you love to this facility. The care here is unacceptable from the caseworkers to the staff. This is one of the worst places you could send a family member. Patients are neglected, and families are rarely kept informed about what is happening with their loved ones.<br><br>Patients’ clothes constantly go missing, and many times they are left wearing other people’s clothing because staff lose the brand-new clothes families bring them. Patients are also often left dirty for long periods of time without proper care or attention. The TVs in the rooms frequently do not work, and at times even the lights do not work, leaving patients laying in darkness. Staff members also fail to assist patients when they need help getting out of bed, sometimes leaving them there for hours without assistance.<br><br>One of the most disturbing incidents was hearing staff make an extremely inappropriate “joke” while lifting a patient with a Hoyer lift. A staff member said, “Is this the same Hoyer lift that one patient fell off from and hit his head and died?” Saying something like that in front of a patient currently being lifted was completely unprofessional, disturbing, and unacceptable.<br><br>It should never take family members threatening to report the facility just to get basic care and attention for patients. Only after families show up in person and say they are going to report the facility do staff suddenly begin looking for missing items or providing assistance.<br><br>The caseworker Lisa has been terrible to deal with and needs to be replaced. This entire facility needs serious changes, accountability, and oversight. The conditions and treatment of patients are unacceptable and raise major concerns about neglect and patient safety.<br><br>This place needs to be thoroughly investigated, and action needs to be taken immediately. I repeat: do not put someone you love in this place.

Our experience working with Somerset Post Acute has been outstanding. From the moment patients arrive, the team demonstrates professionalism, compassion, and a true commitment to quality care. The nursing staff is attentive and knowledgeable, and they consistently go above and beyond to ensure residents feel comfortable and supported during their recovery.<br><br>Communication with families and partnering providers is clear and timely, which makes transitions of care smooth and reassuring. The facility itself is clean, well-maintained, and welcoming.<br><br>Somerset Post Acute truly stands out for their dedication to patient-centered care and positive outcomes. We highly recommend them to anyone in need of skilled nursing or rehabilitation services.

It was a happy place to be. The nurses and staff were attentive, caring. The facility was clean and cheerful. They have students that comes in and assist.<br>Rehab department was wonderful. Upon my admission Christine Garcia and social service Sharon was friendly and made me feel welcome.<br>5 stars to the team and staff. Food is delicious. I recommend this facility.<br><br>Beverly Cone

Friend fell out of bed three times, had half her skull removed due to a brain aneurysm , only 40 years old, recovering and they still wouldn't put up side rails, said it was illegal which is not true, per health department and next 3 nursing homes had them. They took no precautions to fix the problem, lied did not monitor her at all, everytime I went to visit she was halfway off the bed. Didn't follow protocol after falls and send her to hospital until I insisted. Final fall almost killed her, social worker at hospital filed complaint and investigation insued by health department which they covered up and finally she thankfully was moved elsewhere. Nursing homes are not the greatest places but keeping the patient safe should be a priority. If your loved one is a fall risk do not put them in this place or you are at risk of them falling out of bed and they don't care at all, collateral damage to them. You have to be pretty sick to not care about causing permanent damage or death. Bad Karma
