I genuinely don't understand how this place can be rated so high, it deserves a 1 star at best. If you care about your loved ones do not send them here. My grandfather is currently at this nursing home and it is a poor experience to say the least. His belongings are stolen, and the bare minimum is being done to help him. Also, human waste has been placed on the same table that he eats on multiple times. Even the CMS rates this facility a 1 out of 5. Do yourself and your loved ones a favor and please pick somewhere else.

My family member stayed at this facility for about 40 days and almost died over there. He was sent to ER again and got survived. The caring team was very bad and never cared about my family. Every time we were there and requested to talk to a nurse, we were told that nurse was so busy and waited an hour for her to come over. when we requested for medication, the nurse was like "we can give him medication but I want you to understand that he will continue to have symptoms". Then the nurse went away. When my family complained about the gout attack, no one even reviewed his profile for medication and not given him the right medication. We had to complain to the nursing director to get the right medication. This facility did not even care about resident's outside appointments. When we informed them for such appointments that need transportation, they said it's already scheduled. However, the driver did not show up on the appointment date and they did not make a backup arrangement, making my family late for his appointment for 2 hours, almost resulting in cancelation. Moreover, they did not even arrange the transportation back to the facility, expecting family member to take him back. A person who named Anna called and said she was the case manager worrying about him. When he first got admitted to the facility, I called and asked for a case manager. I was told that no case manager was around, only a social worker. It's so difficult to get a hold to the social worker as well. When he was sent to ER and decided not to go back to this facility, all of his belongings were messed up. The facility took away his new cane and walker and unable to locate. Please think 3 times more before you send your loved one to this facility. One star is given to the lovely food service workers who always asks my family to eat and they found out he passed out and rang for the nurse to send him to ER.

Extremely Poor phone system. I am calling for the 4th time this morning the first 2 times was transfer to a fax, when a live person did pick up after 10 minutes on hold I was immediately transferred back to the automated system. They need to straighten out the phone system especially when someone is calling to give important information to the nursing director.

I feel truly blessed to have my husband received exceptional care and support from San Francisco Health Care and Rehab. This facility’s staff: nurses, CNAs and social workers kindness and dedication have made a significant impact on my husband’s well-being. I highly recommend this place to anyone wants the best care!

My mom stayed at San Francisco Health Care Nursing Home at Grove until her passing. I am so grateful to all hard working, compassionate, and attentive staff there: CNAs, nurses, physical therapists. They were taking a good care of my mom. The building itself requires some renovation: all facilities are quite old (this is why I rated it 4 stars).
