My mother has been a resident of initially Sage Meadows assisted-living and in their memory care for the last year and 1/2. She received such loving and tender care from the caregivers. She always felt safe and well cared for. She died a very peaceful and dignified death a few weeks ago at age 99. I will forever be grateful to the exemplary end of life care provided to her by her caregivers, Especially Candy, Patricia, Derrick and the attentive night staff.

My mother loved living at Sage meadow the first two months she was there. Then she got sick and is now filled with anxiety and thinks nobody cares for her there, but I have seen them bend over backwards for her knowing that she was sick and her memory has gone down hill fast! The do what they are supposed to be doing, it may not be her exact timing according to her demands but it gets down. The staff is very helpful and kind and have put up with a lot from her and other people since she has been there, so bless them for still caring and taking care of my mom. The head staff is very knowledgeable and helpful when working through things.

We are so blessed to have found Sage Meadows for our parents. Like many, my siblings and I got a quick education in all things related to caring for our parents as their health declined. Our dad's Community Care Inc. case manager recommended Sage with the warning that past reviews didn't look good, but her current experience was very favorable with the new owner, administration and staff. We appreciated the prompt responses from the administrative staff; they communicated clearly and were very flexible in meeting our needs. Our parents had adjoining rooms which was wonderful for them. Sadly, Mom passed away two weeks into her stay at Sage as she had qualified for hospice just prior to her arrival. We believe that she could let go because she experienced the compassionate care from all the staff, and witnessed how well our dad was treated. I was a CNA for many years, as well as a social worker in a nursing home, so I know that it's a tough job; I expected to find issues or concerns -- but I have not had one in the five months Dad has lived at Sage. My sibs and I are in and out throughout the week; we all agree that Dad is loved and well cared for. He seems quite content and is more social than we've seen him be for several years.

I am happy to post a glowing review for Sage Meadow! My Mom has lived here for over 4 years, back when it was Artisan and the improvement with Sage Meadow and their new management is amazing. My Mom is taken care of with such kindness and attention, my family and I could not be happier. They are so responsive to her needs and our requests. I can sleep easier knowing that Mom is safe amd cares for!

It's was not easy when mom had to move from assisted living to memory care. She was in a place that turned out to be not good at all. We needed to move her from this first memory care facility in a hurry. We toured Sage Meadows of Middleton with the Dawn and Colin. Some times you just know that people really do care. I feel strongly that the staff here is well taken care of and that in turn leads them to take very good care of the residents. We moved mom in the 22nd of August 2020. I have had more communication from Sage in 3 weeks than I did the whole 9 months from the last place. I highly recommend Sage Meadows!
