About Rayville Nursing & Rehabilitation Center

Rayville Nursing & Rehabilitation Center is a 149-bed skilled nursing and long-term care facility in Rayville, Louisiana, with a dedicated memory care unit serving both men and women.

Families consistently praise the staff, describing nurses and caregivers as friendly, personable, and attentive. One family noted the facility offers "a more family-like environment with nurses and staff who know the community and have been there for years." Staff willingness to go above and beyond appears in multiple accounts. However, the physical environment drew mixed reactions, while some found it adequate, one family felt it lacked brightness and airiness.

A significant concern emerged in one account involving a dementia patient admitted for short-term rehabilitation who fell and suffered a hip fracture within hours, raising questions about whether the facility's actual capacity matched its stated readiness for dementia care. This incident contrasts sharply with positive reviews of staff attentiveness.

Not a fan of nursing/rehabilitation facilities as it is. Would love to have had the atmosphere bright, airy and fresh. Just seemed a bit depressing to me. Staff was a joy and helpful though!

Rayville has a high quality star rating! A small nursing home with a smaller memory care unit for males and females. So this is a great facility if you like a more family-like environment with nurses and staff who know the community and have been there for years and great relationship with the hospital and doctors in the area.

We were told that they were equipped for dementia patients for a 20 day rehab. They in fact were not! In the less than 6 hours that he was there, he fell and broke his hip. 0 out of 10 would recommend even temporarily.
