About Prairie Manor Care Center

Prairie Manor Care Center is an assisted living and skilled nursing facility in Blooming Prairie, Minnesota, offering round-the-clock care for seniors. Incorporated in 1966, the nonprofit, church-affiliated center operates two connected facilities: Heather Haus, an assisted living facility with 20 beds, and a skilled nursing facility with 40 beds.

The center provides comprehensive elder care services including physical, occupational, and speech therapy delivered in-house by specialized therapists. These services address conditions ranging from falls and muscle weakness to cognition and swallowing issues. Inpatient rehabilitation is available for residents recovering from hospitalization or injury, while outpatient therapy services are also available to the broader community.

Residents participate in a variety of activities and programs throughout their stay. The facility offers both short-term and long-term care options, with medical services including in-house doctor consultations as part of the care model. Prairie Manor is independently owned and governed locally, though church-affiliated, and serves the Blooming Prairie area and surrounding communities.
