Poplar Care Strategies

Poplar Care Strategies is a skilled nursing and long-term care facility located in Loogootee, Indiana, accepting residents of all diagnoses and functional levels. The facility offers rehab-to-home services with private suites for rehabilitation patients, spousal accommodations, and long-term care options.

The centerpiece of the facility is its therapy gym, one of the largest in Southern Indiana, staffed with a Physical, Occupational, and Speech therapy team that has been with the facility for over 20 years. Therapy services are available up to 7 days a week for skilled nursing patients, long-term care residents, and the community through outpatient services with dedicated parking and waiting areas.

Medical services are provided in-house by Dr. Kiran Burla and Leanna Graber, NP, with additional specialists including audiology, podiatry, dentistry, optometry, and psychology. A full-time licensed social worker is on staff, and the facility provides transportation to and from medical appointments using a facility van. Dietary services are managed by a chef working with a licensed dietitian who makes routine resident visits.

The facility features amenities including a hair salon, courtyard, and garden. Programming emphasizes music, crafts, gardening, games, and community outings. Weekly church services are offered, rooms have individually controlled thermostats, and residents have access to the How's Mom App for family to track medical status and care in real time.

Pets are permitted to visit the facility. The facility accepts Medicare, Medicaid, Anthem, Blue Cross Blue Shield, Humana, United Healthcare, and some VA benefits, with an in-house team to assist with Medicaid applications and updates.
