Residential Living

Assisted Living

At Plymouth Square our 24 hour staff is available to assist residents with activities of daily living as needed. We want our residents to enjoy a carefree lifestyle.

Skilled Nursing

Our nursing services are multifaceted. Our nurses take care of the residents medically, emotionally, and physically. Our licensed nurses manage, observe, and evaluate each resident’s care with compassion and understanding.

Rehabilitation Services

Our restorative nursing and rehabilitation team is made up of registered nurses, licensed practical and vocational nurses, physical and occupational therapists, and speech-language pathologists. All of the members of our rehabilitation team work together to promote independence, safety, and they help each resident attain their highest functional ability.

Social Services

Our social services team seeks to help residents attain their highest practicable physical, mental, emotional, and psychosocial health. We are aware of the worth and uniqueness of each individual, and we treat each resident with respect.

Our social services team creates an atmosphere of support for our residents, and, as desired, we spend a great deal of time working with both residents and their families in difficult transitional periods.

We foster a supportive environment by ensuring each resident has adequate decision making opportunities, providing tours for potential residents and families, assessing psychosocial health, and guiding each care plan.

Beauty and Barber Shop

At Plymouth Square we understand that it can be difficult to get to a beauty or barber shop. In order to make our resident’s lives as easy as possible, we provide beauty and barber shop services on-site. For pricing and appointments please contact the leasing office.

We are dedicated to helping meet the needs of each person who chooses our community to call home.We provide three levels of care at Plymouth Square. We offer residential living, also known as independent living, we help with activities of daily living in our assisted living community, and we provide on-site rehabilitation and restorative nursing to help residents regain their highest level of independence.

We understand that the transition to a new community can be challenging, and we strive to make the process easier through offering church services, live musical entertainment, trips and local excursions, informative educational lectures, fashion shows, and Bingo nights.
