While it is good news that they accept Medicaid the facility is very depressing, dated and dirty. Most of the staff try to be helpful but the communication between departments was not good. I would look at another facility if possible.

my name is Patrick Daly, and I had an infected foot, and I went to Phoenix rehabilitation center and they helped me out tremendously, I'm walking again and I'm getting out of here soon, I want to give a thanks to all the employees and everybody who helped me out.. and the food was good 👍

My buddy is at the Phoenix center and he is so happy there and getting great care from the staff. When I came to visit the administrator was very helpful and friendly. Highly recommended!

I want to Thank Reuven and the staff for taking good care of my mom. For getting back to me quickly if there are any issues. It is a comfort knowing she is safe and comfortable. She can be a handful but use are doing a great job. Thank you
