This FACILITY is SHIT!! Please don't leave your loved ones in here!! Mom's been in her 1 year. Went in for rehabilitation, well that didn't last. Gotta see "their doctor" before can do more therapy. Well NO DOCTOR EVER COMES!! They LEAVE her in her wheelchair days at a time. No changing her. Sits in same diapers for DAYS!!! Had 1 SHOWER since JANUARY!!! Room has mold and no repairs or cleaning up. I've been at facility for hours visiting and ask for someone to come in & change her, always " ok, be right back" 4 hours later, still waiting. I could go on. PLEASE PLEASE THIS PLACE NEEDS TO BE REPORTED & closed! Also looking over reviews- Harborview- Premiere they respond with SAME RESPONSE NEVER TAKING ACCOUNTABILITY

My brother was a patient at Perimeter Rehabilitation for a short period of time before he passed. I have never been fond of nursing homes, but I do understand that it takes real passion and compassion to care for people who can no longer care for themselves. Unfortunately, my experience with this facility left me deeply disappointed. I learned that he passed at 9:43 a.m. Later, when the funeral home arrived at 5:30 p.m. to pick up his body, they informed me that he had been left sitting in a dirty diaper for all those hours. They even had to tell the staff they would not transport him until he was fully cleaned. That morning, when I arrived and cried, prayed, and sat with him for hours, he appeared clean and oiled — but only in the areas visible to us as his family. Hearing from the funeral home that he had been left in his own feces for so long was heartbreaking and unacceptable. (Therefore I can only imagine all of the other days and things we did not know about) After everything he had already endured, he still deserved dignity in his final moments and afterward. I am very disappointed that this was not the care he received.. My times might be off but only by minutes. He was on room 225 and passed away on 3/21/26 #hemattered, #theyallmatter 2 star review is harder to remove ***

My 28 yr old son is presently in this facility. He is nonverbal, can not move his body to reposition himself. We were referred here by the hospital in Decatur. I've wanted to transfer him out of Perimeter since day of arrival. This is day 15. Every day here has been stressful for me as his Mom. Everyday I have had to seek and ask for care for my son. Checking for hygiene. Repositioning. Wound care. Catheter care. It's not a place to bring a 28 yr old nonverbal patient. CNA's are always " I'll be backing, huffing and puffing, and it makes me wonder why they claim to be CNA's. CNA characteristic is push a call light off and I'll be back. If you bring a loved one here.. come everyday to check on them. Still trying to get referrals out but I'm getting feedback that facilities are not receiving the referrals from the case worker here. The case worker has never came to my son's room, or called my phone, to give me any information about the referrals. I know they want to keep my son here but our family wants to go if we can get accepted at another facility, that can give my son the care he needs before coming home. They do have Therapists here who do a amazing job. He gets meds on time, it's the repositioning, and CNA care that is lacking. I see a CNA 1 time doing anything for him from the time I come here at 10:30 a.m - 8pm. And I see them 1 more time because I have to ask to check and reposition before I leave. So he is on one side position 4-5 hrs if i don't ask. Lousy care!

My mom was medically neglected several times throughout her stay at Perimeter. Left on a bed pan for several hours and the reason she left was in an ambulance to the emergency room after a nurse gave her insulin without checking her blood sugar AGAINST her consent. When I followed up, no blood sugar reading was documented on her chart. All of a sudden no staff were legally allowed to talk with me. They promised to follow up with my mom and failed to do so. My mom requested her medical records and they are currently denying access to the records saying she will have to come in person (currently in a physical rehabilitation center) to get them. Purposely making it more difficult to get to likely cover up the lack of documentation they have on her chart. Facility is dirty, old, and smells like urine. Nurses are rude and get an attitude when you ask for help or just ignore the call button. UPDATE: The facility responded to this post and has ignored all contact attempts made. I spent 2 hours sitting down with the director of nursing to discuss this concern where I was provided no clarity on the event and was promised I would receive a follow up call. They never followed up with myself or my mother. Again, do not send your loved one here unless you want them to die or be in far worse condition than you brought them.

Complete trash facility! Not clean they do not take care of the patients at all my aunt has been here for time and has suffered several UTI’s from her not being properly cared for. You call they have attitudes, you try to go there they have attitudes and always “ I haven’t been here” and that “ everything fine” Just for her to be admitted in the ER and were told everything is fine. That don’t make sense. It’s disgusting it’s always dirty floors sticky this place is not good they need to be reported!!
