Please anyone who has in this place a dear adult family friend member and they get sent home because they can supposedly take 30 steps ask them to prove it to you with the patient before they sent them out the door. They sent out my boss nana home and nope she can’t take 30 steps on her own 12 feet away she got exhausted walking from her bed to the toilet with a stroller and she couldn’t get on the sit due to stiffness that got worse and worse as she dragged her feet to complete a step. The therapist was wrong. Always always ask for proof before signing your loved ones from anywhere.

Wow. So many good things to say! Everyone was amazing. From the CNA's to the nurse's, PT and OT, the activities department that kept me sane because I couldn't leave the facility because of my condition and treatment. Daily activities were a great escape from my circumstances and condition. Physical therapy and Occupational therapy worked to keep me moving and making progress. Connor, the head of PT always pushed me and motivated me to do more during therapy than I thought I could and it's helped so much now being out of the facility. The whole team works really hard to make sure you're ready to move around independently once you're discharged. So many amazing people but I have to mention a few by name. Bianca, Ebelyn, and Danielle are amazing CNAs. They don't get enough credit for what they do for their patients every day. Jessica, Cristina, and the other nurses always took care of me. They were very friendly and answered any questions I had and always made it feel like you were walking down the neighborhood saying hi to friends instead of being in a medical facility. Connor, George, Trey, Marcus, Christi, April, Brian, Gabe, and the entire PT/OT staff made therapy fun and challenging while it strengthened us and prepped us to leave when we got better. And last but certainly not least! A HUGE thank you to the Activities Department. Kayla, Serenity, and Vanessa! Like I mentioned, kept me sane. The nail salon they run is worth the five star rating on it's own! Wednesdays @1 PM. Appointment only! Lol. Something fun and creative to do with then every day which was very much appreciated! The fact that I remember all these names is also testament to how well I was treated. Thank you Park Avenue Staff!

Giving a 1 because a zero is not possible. My mother was transferred there from TMC. She is on dialysis and had been battling infection and had been recently diagnosed as diabetic. When I went to visit the first day I was told that she would get physical therapy twice a day to try and build her strength. Only once in a week did they get her out of bed to walk. About 2 days in she developed a horrible rash on her neck. I was told they would have it looked at. I was there every day asking. It gradually got worse. Itching and and she was getting horrible pain. I finally snapped and demanded to speakbwith a charge nurse. I was told they would have to wait till Monday ( it was Sunday) to get dr approval to look at her rash and to get approval to check her blood sugar, they claim to not know she was diabetic. I demanded transport back to TMC. At TMC we were informed it was shingles and as I am looking at her transfer paperwork from this hell hold it clearly said she was type 2 diabetic. Do not take your loved ones here!!!!!

My uncle was a resident for about Eight days, he thought the food was great at least for lunch lunch dinner was a little iffy, but still OK. The facility was clean. They gave baths every Tuesday and Thursday as promised. The only thing that happened to my uncle Richard was that when he went for a bath one day, he forgot to take his eyeglasses off the food tray. The food tray was picked up and he lost his glasses. They could no longer find them. So my uncle Richard is out of the facility now and is making an appointment for new Eyeglasses. he really needs them for seeing the TV and For doing his puzzles and reading books. I wish he did not lose his glasses there. But all in all to say the facility the personnel who took care of my uncle the doctors that came in and the nurses that were excited to take care of the patient was so wonderful as per him when I called frequently every day and asked him how everything was going. He said everything was pleasant and everybody was nice. They take good care of him and he really really enjoyed it especially the Physical Therapy department. The guy that helped him do exercise really impressed him to do more at home so now that he’s home he is trying a little harder to take care of himself and to do exercise and get outside for a little fresh air every day, my uncle Richard and his cat fluffy are doing great as per him. I have not seen him since he got out of the facility and I have not seen him since the last time I was there in Arizona, but he sounds excited which is better than what he used to sound. I love my uncle so much and I hope he stays getting better and not have to go to the hospital again or another facility and if he has to go to a facility, I do hope it is this one thanks for all your help. I am currently a resident of this facility and I have been for 7 weeks. I am here under long-term care. To begin with the management staff has been very accommodating as I still work part time remotely and they have been wonderful in providing me an area to still accomplish this very important activity for me as it keeps me healthy mentally which is just as important as physically. The minor other complaint I had when dealing with my rooms was quickly addressed with complete satisfaction. The physical therapy and occupational therapy is top notch. I have been dealing with a spinal cord injury for 40 years from my car accident and they are aggressive at my own pace and I have shown the most improvement since my accident so very long ago. I personally like the food as I am able to eat in the dining hall which they will take your order of the day and if you do not like what they are having they will give you alternate foods to eat. For the patients who have to eat in their room if the food is cold they have microwaves at the nurses station and will heat food for you. They do allow refrigerators in the rooms which is a big plus also for snacks and such. The call light times could certainly use some improvement but I have seen where they will try to give you a roommate which can balance it out a bit by having an independent person with somebody who needs more care and they can bring to attention but the other person needs. They have many activities daily and once a month they do have a resident council meeting with the staff and the residents to address any problems. My overall experience has been very positive and I am critical of staff that do not help people who have no voice literally or mentally or for whatever reason. The majority of CNAs and nurses have compassion that I admire in their positions. The CNAs are the backbone of patient care as they are in any facility and they do a great job overall. I am not sure what has changed in the last year again upon reading the reviews but this is just my opinion if anybody needs to read something about this facility. They also allow smoking which of course we all know none of us should do but I do and it is allowed here. I was also able to decorate my room and family is ALWAYS welcomed.
