Paradigm at the Brazos is a 56-bed skilled nursing and long-term care facility in Richmond, Texas. The community serves patients recovering from acute injuries, those needing extended care, and individuals with complex medical needs.
Families consistently praise the clinical and personal attention from nursing staff, therapists, and aides. Multiple reviewers highlighted the responsiveness of nurses and certified nursing assistants, calling out the physical therapy department as particularly strong. Administrative staff members earned specific recognition for their availability and care. One family noted the activity director and kitchen staff for their dedication and well-seasoned meals. A long-term resident of 10 years credited consistent physician and nursing relationships built over years of care.
However, reviewers raised serious concerns about the facility's physical condition. Multiple families described cracked flooring, rusted equipment, and persistent maintenance issues that staff work to manage without full success. Some reported inadequate supervision and positioning protocols. One family described a discharge they felt was unsafe; another raised a critical incident involving cardiopulmonary response. These accounts stand in sharp contrast to the clinical praise and suggest gaps between care quality and operational oversight.
Paradigm at the Brazos holds a 3.8 out of 5 Google rating based on 16 reviews. Prospective families should weigh strong staff commitment against documented facility conditions and the need to verify current care protocols and management responsiveness.

If I could give negative I would my mom was here just over 2 weeks and is still fighting the bed sore! They discharge her unsafe because I insisted on a discharge. The caregivers are overworked and probably underpaid. All they care about is filling up the beds. After therapy my mom just laid in the bed or chair all day, which is how she got the bedsore. Don’t leave your family there unless they can care for themselves

I have lived in this nursing home for 10 years. The name and ownership has changed recently but most of the staff has worked here for decades. With few exceptions, I have been treated by the same doctors and nurses since I was admitted, when I was suddenly homeless and suicidal. I am surprised to read some of negative comments I have found here, most of which I find superficial. No place is perfect. I have had my own concerns and complaints. Right now, the facility is out of paper towels and toilet paper, at least in our room. But I am quite sure this will quickly be rectified. This sort of thing happens in the best of brand new homes with perfect floors. I want to put in a good word or two that negates some of bad reviews I have read this morning.
