My 94 year old mother was placed here a few months ago. I visited my sister who lives in Florida, I live in Virginia and wanted to see this facility. It does need some updates, but I was very pleased. This place is very clean, friendly staff and the best care given to my mom. Lori has been awsome with all the paperwork and being so friendly.

My companion was brought to Oakpark Health & Rehab May 19, 2026, and not to happy about it. When I got there today I was so relieved. Carlogio was happy. Everyone was so helpful, a refreshing scent upon entering. Exceptionally clean. I will definitely sleep tonight. Thank You! Everyone. Dawn Lewis

My husband was admitted to Oakpark for rehab after open heart triple bypass surgery. He received Physical, Occupational and Speech Therapy. He was severely weakened after the surgery. Physical Therapy worked diligently with him to safely reach independence with a walker. Occupational Therapy greatly contributed by emphasizing both upper and lower body strengthening which especially protected his internal surgical site promoting healing. Speech Therapy quickly detected the need for work on his recall and memory related to Parkinson’s Disease affected by post operative side effects. The nursing staff especially Adam, Ben and wound care nurse, LuLu, were professional, attentive, and proactive employing critical thinking in many aspects. Myla, the supervisor, was an invaluable resource as she was empathetic, attentive and always available addressing and ensuring all my questions and concerns were answered in a timely manner. Administration from Su Soriano to Social Services instrumented a seamless transition from admission to discharge.

The food is awful. Like bad hospital food. The dining room was freezing on several occasions and instead of the staff raising the temperature (vs keeping the AC on high) they went out and came back in with sweaters on while we all were freezing while trying to eat lunch. It appears the staff to patient ratio is too low. Pushing the call button for a nurse does not result in any type of reasonale response. Plus, some patients cant even find the button to push and are left alone for long periods. Room, floor, tray, were all mostly dirty while we were there for a week. Some Nurse Assistants were better than others - most weren't very servcie oriented - they just did the bare minumum. Therapy schedule was all over the place - never knew what day or time my mom was getting therapy unless I asked. Not sure why the Doctor didnt make more/frequent visits to the room to help determine the proper medical treatment for her as her systoms changed each day. Both Wound Care nureses were very good - they seemed to care and knew what they were doing. Rest of the staff came across as either overworked, slow moving, doing the bare minimum given their workload. Found my mother in a wheelchair in the hallway unattended twice. She was there for a week - begged me to take her home each day -- and pased away 7 days after I was finally able to get her discharged. It was an awful experince for me, my mom, and my family.

My mother was there for rehab and I have to say the staff was very attentive and very nice. The case worker Brittney was so very helpful and very professional , after reading most of the reviews on here, I truly have to say it was a very good experience and my mother got the care she needed. Thank You
