My father passed away at this facility on 2/11/25. They actually called me from HIS personal phone to tell me he died. After not hearing from him for nearly a week, I was excited to see his name pop up, only to be met by a cold-hearted voice that still shakes me to this day. Without offering a single word of condolence, the nurse immediately demanded to know where to take his body. When I went to pick up his belongings, not a single staff member said a word to me. My dad had already expressed numerous complaints about several of the nurses caring for him. While he wasn't always an easy resident, compassionate care is the job they signed up for. Families work their entire lives to afford care for their loved ones. Do not place yours here. Find a facility that hires staff who actually have a heart for their patients, and strong leadership that will stand for quality care.

Excellent care. All associates friendly. Food is very good. Sometimes a little warming needed. They always warm it up for me. OT & PT are great.

What a foul place!! Very neglectful! Let people sit in wet or dirty diapers. My mother was visiting friend yesterday and in the 6 hours she was there, nobody came in to take care of the patient! They need people who enjoy taking care of people. This place should be closed down. I don't think it even deserves 1 star!

I visit a patient here whom ive become friends with for 3 years. I originally met her as a volunteer. She’s been at this rehabilitation center for a little over a year. This is by far the most exceptional facility ive visited. ( unfortunately, i have other friends at rehabs) The 24/7 care of different kinds of staffers is amazing. And the staff are superb. From the volunteers, house keepers, cooks and nurses. May God bless these ppl for caring for our sick, elderly & loners… THANK YOU ALL @ OAK
