My mom was at NP for 2 weeks in July 2026. The facility is very clean- the staff is very friendly-<br>We did have one issue from a night nurse who forgot her morning antibiotics.. they took care of that right away. A big shout out to Chelsea, Sonya, the entire PT/OT department! There were many other fantastic employees. I would put it on your list to visit- my sisters and I were there a lot so were very present during her stay-

The staff is incredibly friendly, welcoming, and genuinely caring. The facility is exceptionally clean and well maintained, and the beautiful private rooms create a comfortable, home-like environment. I would highly recommend this skilled nursing facility to anyone looking for quality care for a loved one.

This facility is excellent. My mother was there approximately 2 weeks and she thought it was the best. Everyone was very professional and attentive to all her needs. My mother and I do not do surveys however my mother was very adamant that I do a survey on this facility to let everyone know that they were wonderful and everyone should recommend their family go here if they need rehabilitation.

Recently I stayed at NorthPark Health and Rehab for 20 days after a hospital visit. There were so many positive attributes and experiences. If in need of this type of facility, I strongly encourage anyone to choose NorthPark Health and Rehab. First, the care staff was outstanding. Their professional, kind, and friendly manners helped to create a place for healing. No matter how private or nominal the task, they maintained their sense of truly caring for me. The nursing staff (Chelsea, Tameka, and John) took care of me with excellence. Furthermore, the CNAs were off the chart! Each person had her/his own way of encouraging my healing. Sonya and Lupe lovingly pushed me to the next step, yet were always there to lend a helping hand. Mya, Joselyn, Alondra, Miya displayed kindness at each moment; they became my angels in this healing process. Lee and Freddy were the angels in the night who saved me from mishaps. Each of these caregivers supported me to feel safe and positive during this difficult time. The physical and occupational therapy staff recognized my needs, growth, and terror toward my physical goals. Jen, Josh, and Chivvon were incredible. Susie had a magical way of always appearing with grace and cheer whenever a need arose. But David was the knight in shining armor I needed to guide me through my physical triumphs. I am eternal grateful to each of them. Two other people of excellence were Francisco and Yanirys Franciso maintained the facility and was always moving to make things right. When the clock in my room broke, he fixed it within minutes. It was amazing how his optimistic nature set the tone throughout the facility. Plus, Yanirys was another kindred spirit She kept my room and bathroom spotless with a smile and a laugh. I began looking forward to Yanirys visits to my room each day. Finally, you might want to know about the food. Ed oversaw the process of feeding us all. It reminded me of conference food at a nice hotel, like a Marriott. A well-balanced diet was served (with fresh fruit and vegetables) and it tasted so good. Ed’s specialties included his cheese enchiladas, fried chicken, ribs, soups, oven fried fish, and bratwurst. If I didn’t like what was on the menu, there was a list of substitutions I could pick. It was clear from the quality and taste of these meals; this kitchen was run with pride and care. The facility prides itself with a pleasant environment in all places. The single rooms with a bathroom to include a walk-in shower was greatly appreciated. The bed was comfortable and recliner a hiatus after PT/OT. I do have three complaints about the NorthPark Health and Rehab facility. One, the night staff needs additional personnel (CNAs) to adequately meet patient support and care. Thinking the night is quiet, since patients are asleep is a misnomer. Nights are when the chaos hits and only one CNA is staffed to manage it (ratio 1:18). My second complaint involves the case manager giving untimely and incorrect information regarding discharge which resulted in less than the 72 hours to complete an appeal. Finally, my complaint concerns feedback. As required, there was a grievance folder to give feedback on negative experiences. But I didn’t have a negative experience – my experience was overall highly positive. Hence, the management needs to add a compliment folder above the grievance. It would only cost the paper, manilla folder, and the time to slip the sheets into the recipient’s mailbox. Imagine the joy for the employee receiving the compliment, plus the increase to overall morale. (Remember it is crucial to always keep this process only positive for all.) During my stay, I requested to speak to the director numerous times about my idea. He never responded or even send a representative In summary, my stay at NorthPark Health and Rehab promoted healing, strengthen, and hope. I strongly recommend people remembering Northpark if they are ever in need.
