About Mt. Ogden Health & Rehabilitation Center

Mt. Ogden Health & Rehabilitation Center is a skilled nursing and long-term care facility in Washington Terrace, Utah, licensed for 108 residents. It serves patients requiring medical assistance, rehabilitation after illness or injury, and ongoing nursing care.

Families consistently praise the physical therapy team and the quality of care for residents with complex medical needs, including long-term stays. The kitchen staff and overall cleanliness draw repeated mention, one family noted the food is "wonderful compared to many other facilities." Staff members are described as treating residents with respect and attentiveness over years of care. However, one recent family raised serious concerns about communication: their husband was transferred to the hospital for sepsis, but the family was not notified until seven hours after admission, pointing to potential gaps in nurse staffing and incident protocols.

The facility holds a 3.5 out of 5 Google rating based on 125 reviews. While most reviews are highly positive and highlight genuine long-term relationships between staff and families, the communication failure documented in the lower-rated review suggests variability in care standards worth discussing directly with management before admission.

My husband was here for 35 days too long. Their PT staff is great! Dan and Niko took great care of him. He was sent to the ER for sepsis and I wasn't notified until he was admitted to the hospital, 7 hours later. Negligent at best! Management needs a makeover nurses are way understaffed if they can't call a family to let them know their loved one was sent to the hospital.

We have and had family members in this facility and it doesn’t deserve the under 5 star rating it holds. The people that rate this facility under 4 stars need to experience the many Skilled Nursing Facilities located from Weber County to Davis County for a significant period of time, which we have dealt with over the years with our advanced age seniors and quadriplegic and paraplegic family members before forming an opinion on the level of care provided by MT Ogden Health and Rehabilitation Center. Tour all of the other facilities and have a family member stay in other facilities and you will see that MT Ogden Health and Rehabilitation Center is above and beyond the standard facility in the Weber and Davis County Area. Thank you MT Ogden for caring for human beings that are in need of Rehabilitation and Skilled Nursing Care.

This place is so kind. If you love your loved ones and can't help them at your home because they need medical assistance please bring to this place. It is clean, It has in there medicine and that is about all it has going for them. During my entire time visited left maybe some hours. They stayed and rehab patients to the point to getting back to their usual activities. I only saw with physical therapy. The heart patients are in there with own interaction or physical patients have time to sit and interact with another human been and listen to non stop conversations. Some people where they are exhausted and getting zero rest. Because of the Modern life style, here they can be relax. Here you are part of bingo, activities out of your room like gardening. Or Been part of the celebration as holidays approach Here you can find staff anywhere in sight the patients willing to be around it. As the patients here they know just sitting unattended for some family members and even without medications on top, just to waiting to inherited in their own home. Here are safe, enjoy the food and living with dignity.

This facility does an amazing job at taking care of people/patients. My elder parent was there 2 different times in the last 6 years. Went home after terrific physical therapy. They loved the place compared to others they have stayed in. They saved another immediate family members life. Everyone says how wonderful the food is compared to many other facilities. Never smells bad in this place. If you are looking for long or short term care solutions. Save yourself heartaches and stress. You will want to make this your place of choice for yourself or a loved one.
