If I could give this place less than one star. I would. The nurses do not communicate with each other about patient needs, from shift to shift. Every time I went in and asked questions about my mother's health, progress, plan or anything whatsoever, the nurse on shift didn't have an answer. My mother had horrible pneumonia, Dr put in an order for her to receive breathing treatments wvwry 4 hours "as needed". They expected my sick mother, with dementia to ask for the breathing treatments if she wanted them every 4 hours. Even though there was no clock in the room and My mother's memory wouldn't allow her to even remember to do it. This place was awful.

After having two relatives live here I can say there is a serious issue with medication care. Your loved one may come in with prescriptions that tells them how much to give them and how often right on the bottle. They WILL NOT administer that medication until they get order from the doctor to do so. They will let them go indefinitely without their medication if they don’t think they have the paperwork. Do your due diligence and request the doctors order be sent over and hold their feet to the fire to prove they are following the orders.

My Mom's stay in the rehabilitation was from 9/9/23 through 9/22/23. The Staff were so helpful and lovely... ALL of them! They pitched in where they were needed whether it was their department or not. They had an "I CARE" attitude. They were so gentle with her, I felt like they were family. I thank Jesus for each and everyone of them. We set goals during the family meetings and they helped us meet those goals. They helped us understand every step of the way, from finances to home care... When God closes a door, don't be upset, because He will open the right door and I believe Heartsworth was the right door. A special thank you and God bless each one of you... Michelle, Fawn, Carleen, Heath, Kendall, Mike, Liza and so many more. God's assistant angel's

My mother is safe and well cared for by an awesome staff. I have had nothing but peace of mind w'her being there. Dad passed Labor Day of '19... they did everything possible for his comfort while accommodating the family. I've unabashedly recommended this facility (and still will) to folks seeking excellent care for their loved ones!!

This is the ABSOLUTE BEST Nursing home! All staff members from Teri as the go to staff member to meet with to enter your loved one to Megan, the staff member who works with Medicaid, to the most caring Nurses, CNA's, the support staff ...... just EVERYONE at this Nursing Home care about your loved one and their families. The Facility is clean and warm with the perfect staff, a cat that lives there to make it pleasurable for the patients and our Seniors living there to the beautiful bird cage add such a warm touch. I thank those wonderful girls who took care of my Mother in Law as they have such HEART for the Seniors and all That reside there. I want to also thank Compassus as they did such a wonderful job with her. They care..... and I could go on and on with this WONDERFUL Nursing home. The BEST place for your loved one. Thanking them is not enough. I plan to write a letter to the Corporate office as these staff members deserve much more than I can say here. They are beautiful souls....each and everyone of them.
