Fort Bayard Medical Center

Fort Bayard Medical Center is a 200-bed long-term care facility providing skilled nursing and intermediate care in Santa Clara, New Mexico. The facility also operates a 40-bed Veterans Home neighborhood for honorably discharged veterans and Gold Star Parents, as well as Yucca Lodge, an 18-bed chemical dependency treatment center on campus.

The center serves residents with acute and chronic diagnoses including behavioral and organic brain problems, personality disorders, diabetes, heart disease, cerebrovascular disease, chronic obstructive pulmonary disease, and atherosclerotic cardiovascular disease. Care services include full nursing care with in-house physicians, social services, activity therapy, physical and occupational therapy, speech and language therapy, restorative care, case management, laboratory services, pharmacy, and transportation.

The facility is located at 41 Fort Bayard Road on the grounds of the historic Fort Bayard site, which dates to 1866 and served as an Army hospital and later Veterans Administration hospital before the State of New Mexico assumed responsibility for the facility in 1965. Fort Bayard Medical Center accepts Medicare, Medicaid, private insurance, and private pay for services.

A new single-story facility opened in November 2010, featuring safer outdoor areas for residents, X-ray rooms, a dental clinic, meditation room, salon and spa, library, computer lab, and a separate wing for residents with Alzheimer's disease.
