My mother has been at 5 points for 2 yrs and there is nothing but praise for Mona Lisa and the administration, nursing staff and director, aides, physical therapist, activity director and what would I do without the guidance and assistance of Angie the social worker. I highly recommend this facility and have.peace.of mind that my mom's being cared for as we did at home. We as family members should assist our loved ones whenever we have.time. No location is perfect. Even the most expensive and private facilities have mishaps. We're all human.

Having a love one in a skilled nursing home will never be perfect but I have the utmost respect and appreciation for the Five Point team! Thank you for everything you do!! Couple shout outs. Monalisa, you are just awesome! You have been there from the beginning, our admission process, to adjusting to the facility and even on billing/accounting…you go above and beyond your title! Angie, thank you for maintaining your clear communication, scheduled care meetings and being the voice for my mom and I. Last, thank you to the medical team, med techs and the CNAs who are always taking care of my mom. Words can’t express my gratitude!
