About Five Points Nursing and Rehabilitation

Five Points Nursing and Rehabilitation is a skilled nursing and long-term care facility in Amarillo, Texas, with a licensed capacity of 120 residents. The community serves patients transitioning from hospital stays who need rehabilitation or ongoing skilled care.

Families consistently praise the facility's staff and leadership across reviews. One family noted that the Director "has been solid from day one" and is "honest, reliable, and follows through," while the Activities Director brings "energy" and genuine engagement with residents. Multiple families highlight caring, professional nursing staff who go "out of their way to get the job done." The physical environment draws positive mention, bright halls, a courtyard with outdoor space, and an open visitation policy that helps families stay connected.

The facility shows some inconsistency in care quality. While many families report smooth admissions and strong rehabilitative outcomes, a minority describe significant concerns around therapy scheduling delays, documentation accuracy in physical therapy progress, and wound care. One family wrote that their loved one was "walking better the day after surgery than she was after 20 days" at the facility.

Five Points holds a 4.5 out of 5 rating across 163 Google reviews, reflecting broadly positive sentiment tempered by occasional serious care lapses. Families considering the community should weigh consistent praise for staff relationships and facility upkeep against documented gaps in therapy coordination and clinical oversight.

Great facility. My husbands care was top notch. Director was helpful and kind. Only thing was housekeeping didn’t sweep .

If you value great care and prompt responses, do not put your loved one here. We had our LO here for only 3 days before he passed and more than one of the nurses needs to consider a career change. It looks beautiful but is definitely lacking in the important things.

I want to take a moment to recognize the amazing staff at 5 Points Nursing Home who have truly made a difference in my mom’s care. First, Christian (Director) has been solid from day one. He’s honest, reliable, and follows through on everything I’ve ever asked. That kind of leadership matters, and it shows. Amber, the Activities Director, is absolutely incredible. Her energy, her love for the residents, and how involved she is… you can tell this isn’t just a job for her. She goes above and beyond, even making sure my mom’s monthly needs are taken care of. She’s truly one of a kind. Jaime, the Charge Nurse, deserves major respect as well. My mom had a stroke and struggles to communicate, but Jaime takes the time to understand her. She makes sure all of her medical needs are handled, and anytime we need something, she gets it done. That peace of mind means everything. The facility itself is clean, and the food is good. Overall, during the weekdays, the care and attention are exactly what you would hope for. Now, I do want to be honest about one concern. On the weekends, there’s a noticeable drop in quality. It often feels understaffed or less attentive. Floors aren’t as clean, it’s harder to find staff, and some residents don’t look as well cared for. I understand student nurses are part of the process, and that’s expected—but the weekend nursing staff needs to step it up. There’s too much standing around and not enough attention to detail. I’m giving this review because I truly see the potential here. The weekday team proves what this place is capable of. If that same level of care carried into the weekends, this would easily be a top-tier facility. Thank you to the staff who show up and truly care—you don’t go unnoticed.
