Essentia Health Grace Home

Essentia Health Grace Home is a long-term care center offering 24-hour skilled nursing in Graceville, Minnesota. The facility provides private and double rooms where residents can bring personal belongings, with serene views of farmland and Lake Toqua visible from many windows.

The center offers a range of services including regular visits from providers for complex medical needs, hospice care with compassionate end-of-life support and skilled nursing, and restorative programming to keep residents active through exercises following therapy sessions.

Grace Home features onsite amenities including a beauty shop, chapel, and outdoor spaces. The facility is located adjacent to Essentia Health hospital and clinic, and sits next to Holy Trinity Hospital and Graceville Clinic.

Originally built by the City of Graceville in 1976 as a 40-bed skilled nursing home, Grace Home was managed by the Franciscan Sisters of Rochester before being purchased by the Missionary Benedictine Sisters in 1996. Essentia Health acquired the facility in October 2010.

Parking is available in front of the building off West 2nd Street. The community welcomes volunteers to help organize activities and visit residents.
